If you wish to be in the running to win these awards, you must fill out a form to participate.

You may enter up to TWO separate categories.

You are welcome to not participate in these categories if you don't wish but if you'd like to, you have to at least be participating in one of the main categories/fandoms.

We will only be accepting five entries for each.
