His eyes met mine and he pulled a piece of paper out and approached me. He asked,"Do you mind filling this out?"

I shrugged and looked at the sheet in his hand. I quickly noticed that he had a ring on his finger. I took the paper and he held out his hand.  I hesitantly grabbed it and he took me to an empty room.  He handed me a pen. I sat at the table and looked over the questions.
1. How old are you
I wrote down 17. Don't ask how I got into the club. All I know is that Dianna said something to the bouncer and she pushed me through.
2. Do you live with your parents?
I wrote down yes and looked up at the man. 
3. Are you single?
I wrote down yes and looked at how many questions were on the paper.  Thank god it's only 10.
4. Do you like older men?
I blushed and wrote yes and on to the next question.
5. How many guys have you slept with?
To embarrassed to answer that I'm a virgin I left the rest blank since they were asking about my sex life. I handed the sheet to the man. He frowned and spoke with a unsure tone in his voice, "You left some blank?" I stuttered out, "I can't answer those questions. If you would like I can get my friends to fill out the form. They are way more experienced then me." Only seconds after I finished saying that it clicked into his head.  A lady walked into the room and asked,"Is this her?"
