Chapter Nineteen
"Let's say you're standing in an OR staring at an aneurysm that's embedded deep into a patient's frontal lobe. There are three things you'll need to remove it. You'll need confidence. You'll need an 11 blade. And some really good instincts. There are some feelings that refuse to go away. They're little distractions, whispering in your ear. Some things just get under your skin. Try as you might. You can't ignore your instincts. It's like they say. Always follow your intuition"

Alex and Lucy were in bed, facing away from each other, eyes wide open. Alex turned around and she did so a few moments later. They stared at each other. He opened his mouth as if he was about to say something but then Mia started crying. He sighed, closing his eyes in defeat. "I'll get her", he said softly, putting his hand on her arm before getting up. She moved to lay on her back and massaged her temple.
Aurora was on top of Owen and she kissed him before getting off and laying on her back. "Good morning", he said.
"I'd say that, yeah"
His phone went off and he got up to check it. "Gas tanker slammed into an SUV on the I-5. Caused a huge pile up. No telling how many traumas are coming in. We should...". He turned around but she was already leaving. "Get going"
Alex and Lucy walked into the ER together. They saw Jack with a patient and he waved at them. Lucy looked away and quickly walked off. Alex glared at him.
Aurora walked into the ER, wearing a trauma gown. A woman walked up to her. "Dr Sloan, right?" the woman said. "You saw us the other night. I'm Casey Hedges. That's my son Parker".
"Right. Is he still sick?"
"Well, his fever was down when I left for my shift. I'm a waitress. But then my neighbour called. She said it spiked again, and then when I got home, he had this rash"
"You've been giving him the meds, lots of fluids?"
"Yeah, but his fever's still above 103, and it's been like that for over a week now. And I mean, I know that you told me it's a virus..."
"Probably adenovirus. The fever can last at least five days with it. It's not that unusual"
"I've been tracking the spikes in his fever, and I... I looked up his symptoms online and wrote down a bunch of stuff"
"Roseola, shingles, lupus"
"I just... I can't explain it, but I know my kid. This is not a virus. I just feel it"
"Okay. I'll go get your chart"
Owen, Derek, Bailey, Shane, Cristina and Beck went out to the ambulance bay as a bunch of ambulances arrived. "Okay, no open flames, no smokers, no use of cauteries", Owen ordered. "Be safe, people". "Paul Dawson, 45. Restrained driver with major intrusion into his area of the vehicle", the paramedic explained. "Bruises to the chest. BP 90 over 60. Pulse 120". "Where's... Where's my son? Where's Ethan?" Paul asked. "He's on his way. Don't worry", Owen reassured him. Beck and Cristina took him inside.
"So, Mr Dawson, there's a little fluid around your lungs", Cristina said. "I'm a little concerned, so we're gonna take you up to CT". She went over to Owen. "Widened mediastinum", she said. "I'm betting his aorta is shredded beef". "This is the patient's son", Owen said, pointing to the little boy beside him.
"Oh, sorry. We gotta take him up to CT"
Aurora was taking care of a patient in the ER when she saw April's patient start to decline. "April, she's bleeding", Aurora said, rushing over. "What?" April said, checking.
"Cord's prolapsed. We need to secure it"
"What is happening?" Elyse asked. "Your baby's umbilical cord has dropped, and it's now trapped against her body, and that's why she's in distress", April explained. "Ma'am, we need to keep the cord inside so your baby doesn't lose any more oxygen", Aurora added. "To do that, I'm gonna have to place my hand inside. I'm really sorry, but it's gonna hurt a lot, okay? Someone page Alex Karev please". "Did you call my husband? Where is my husband?" Elyse said. "I did. I called him. Hold my hand", April replied.
"Okay"
"Okay?"
"Okay". She screamed in pain.
"We need to get this baby out. Page Alex Karev again. Alright, where's OB?" Aurora said. "Tell them to meet us in the OR now!" "Elyse, we're going to surgery right now to take care of you and your baby", April said. "Okay?"
"Please don't be surgical. Please don't be surgical", Callie muttered, looking at her phone as she and Lucy entered the ER. "It's surgical", Lucy stated, looking around. "Hey, there you are," Arizona said. "Oh, no. Me first", Bailey said. "You know we're both attendings, right?" Lucy said.
"Right. I forgot. Either one of you will do then"
"Great. Thanks, Bailey. What do you got?"
"A bike messenger caught in the blast"
"Shepherd, can you page Dr Garrity?" Callie asked. "My plane for TED leaves in four hours".
"The frat party for fancy people"
"You'll be singing a different tune when you're there next year"
"Map my genomes. That sounds like fun", Arizona said. "It's not a spa treatment. It's science", Bailey corrected. "Just like building new cartilage", Callie said. "As I say in my speech...". "Okay, Callie", Lucy interjected. "Callie. The patient".
"No, you're right. I should keep it fresh for the conference anyway"
Cristina and Beck were operating on Paul Dawson when Owen came in. "Just looking for a status report for the patient's kid", he said. "The injury extends into the aortic arch", Cristina explained. "So in order to maintain head perfusion, I have to bypass in two circuits. We're just waiting on Russell for a second opinion".
"You had to call Russell for this?"
"Not really. But ever since I became his boss, I like to make him think that he's still mine. 3-0 prolene"
"So could I give the kid a time frame?"
"Give us an hour. We'll know better then"
"Okay"
"And the baby's out", Dr Meyers informed them in Elyse's surgery. "Okay, let's begin to explore the abdomen", April said as Alex and Aurora took the baby.
"Oh, God. Elyse. She's losing too much blood. More lap pads. Now!"
In the NICU, Alex and Aurora took care of Elyse's baby. "The baby's dad arrived. A nurse is bringing him up", Aurora said.
"Which nurse?"
"Um, Thompson, I think. Why?"
He shrugged.
"Don't tell me you're still on this whole "bad feeling" thing"
"Today, me and Lucy, we walk into the ER, Jack waves and she just looks away and walks off. Come on, that's weird, right?"
"That's a little weird"
"Not just a little. A couple of days ago, I saw them coming out of a supply closet together"
"Okay, you should've led with that. That's a lot more weird"
"Sorry. I'm kinda all over the place right now"
"Do you think there's something going on between them?"
"I don't know what to think anymore. She's just not the same. She hasn't been since Mia came along"
Before they could say anything else, Jack entered the NICU with the baby's father. "Vince, this is Dr Karev and Dr Sloan", Jack said. "They've been taking care of your little girl". "Look at her. She's... She's perfect", Vince said. "She's doing really well", Aurora said. "And we spoke to Dr Kepner", Alex added. "Your wife is still in surgery. We should have an update soon". "Thank you. Thank you very much", Vince said. "Your baby's pretty darn cute", Jack commented. "Want to hold her?" Jack picked the baby up and handed her to Vince.
Owen and Aurora were in the on-call room together, he was lying on his side with his head in her lap. "The kid is pretty freaked out right now", he said.
"You were good to stay with him"
"He needed someone"
She smiled and then yawned. "I have to be back in like a half hour. Should I sleep or should I shower? I could sleep in the shower. But I'm also starving. Have you eaten?" She looked down at him to see that he had fallen asleep. She smiled softly and leaned back against the wall behind her.
Paul was crashing, Beck was giving CPR and Cristina was ready with the paddles. "Charge to 200", she said. "Clear". They watched the monitor and Beck felt his pulse. "Got him. Got him", she said.
Jackson walked into an empty hallway, finding Beck sitting on a gurney. "Hey, you lose your patient, too?" he asked, sitting down beside her.
She shook her head numbly.
"You okay?"
She shrugged.
"Beck"
"Everything's falling apart. This isn't the way everything was supposed to turn out. It's... It's not right. I'm so alone. Aurora and I aren't even speaking. And maybe I shouldn't be saying this to you but I still think about the abortion. And it hurts. A lot. For many reasons that I won't bother you with"
"Okay"
She started crying.
"Hey". He put his arm around her and her head fell onto his shoulder. He rubbed her arm comfortingly as she sobbed.
She sighed, sitting up and their eyes locked. His eyes fell to her lips. "Hey", Cristina said from the doorway and they looked away from each other. "We got to go do the hypothermia on Paul". "Right", Beck said, sniffling and wiping her eyes.
"Do you need a minute?"
"No. Not at all"
Cristina and Beck covered Paul with ice pads while Meredith talked to them about her patient. "Could it be Munchausen by proxy?" Cristina suggested. "What? That she's making her own kid sick?" Meredith questioned. "No. She just has a strong feeling that it's something more than strep". "Or she's crazy", Beck said. "What's his core temp?" Cristina asked.
"Still 93"
"I mean, it's strep, right?" Meredith said. "If it walks like a duck and quacks like a duck..."
"Mer, the duck's strep test positive", Cristina interjected. "What happened?" Owen said, entering the room.
"He had an MI. He coded"
"Why didn't you page me?"
"Why would I? He's my patient"
"So what are you doing?"
"Therapeutic hypothermia, just 24 hours. We're cooling his body so he'll avoid another attack"
"But he just had surgery. This could increase his risk of infection of bleeding"
"Or it could save his life", Beck said.
"Did Russell agree to this?"
"You know, I don't know. I didn't ask him", Cristina replied. "'Cause he's my patient".
He walked out.
"I think he's gonna tattle on you", Beck said.
Beck was watching Paul. Owen came over to her. "I talked to Russell", he said. "And he agrees with Yang's course of hypothermia, although I do think he's only agreeing because he's afraid of her".
"Yeah, probably"
"He has a little boy, who is scared to death that both of his parents..."
"You don't think I know that? He has a kid so do most of the patients we treat. We're not supposed to get emotionally invested"
"I know that"
"Do you?"
He looked down.
"Whatever. I honestly don't care. I have had a crappy day and I really don't need you making it worse"
"You should talk to Aurora"
"She doesn't wanna talk to me, no thanks to you"
"She needs you. It's got nothing to do with me"
Beck entered her and Aurora's apartment where Aurora was sitting on the couch, watching TV. "Hey", Beck greeted, announcing her presence.
"Hi". Aurora looked surprised and turned off the TV, sitting up. "You're back"
"Yeah. I've been sleeping in my car"
"Beck..."
"I know. I'm so sorry. I don't know why I let it get like this. I don't know why I can't just support you. I really wish I was different"
"I don't wish you were different, Beck. I didn't mean anything I said. I am so sorry. But I love him and you have to respect that"
"I know. It's just hard. I just want you to be happy"
"I am"
"Okay"
"What happened?"
"I just... If I was different, I could let go. I could let myself be with him. I could be happy"
Aurora got up and hugged her tightly.
