Frisk is a literal reincarnation of Chara. We never see them fall into the underground; they just are suddenly over Chara's grave. It would also explain the severely alike appearance. Chara's ghost is still around, though, even,though Frisk is them. Like a Kagome and Kikyo sort of thing.
