Chapter Fourteen
"One of the hardest lessons as a doctor is learning to prioritise. We're trained to do all we can to save life and limb. But if cutting off a limb means saving a life, we learn to do it without hesitation. It's not an easy lesson to learn, and it always comes down to one question. What are the stakes? What do we stand to gain or lose? At the end of the day, we're just gamblers trying not to bet the farm. Surgery is a high-stakes game. And no matter how high the stakes, sooner or later, you're just gonna have to go with your gut. And maybe, just maybe, that'll take you right where you were meant to be in the first place"

The residents were getting ready in the residents' lounge while Meredith complained about which clinical trial she should choose. "It's horrible", Meredith complained. "It's the opposite of horrible", Lucy replied.
"Alzheimer's or diabetes?"
"If you continue to whine about how you can't decide which ground-breaking medical advancement to put your name on, I will punch you in the ear", Cristina said.
"Well, my name's already on one of them, according to the chief. I mean, diabetes..."
"Affects 240 million people worldwide", Lexie finished.
"And Alzheimer's is..."
"The seventh leading cause of death"
"It's Sophie Choice", Lucy remarked. "It's Sophie's Choice", Meredith agreed. "I've never seen that movie", Aurora said. "You should. It's really funny", Beck said. Bailey entered the room. "Greys. How long has your father been having abdominal pain?" she asked. "Abdominal pain?" Meredith asked.
"It sounds like abdominal pain. Thatcher Grey is not an easy man to get information from"
"Our father's here in the hospital?" Lexie asked.
"Yeah, I admitted him an hour ago. You didn't know he was coming?"
"What's wrong? Could it be related to the liver transplant?"
"Of course, he's rejecting it because it's my liver,'' Meredith commented.
"Ok, look, just do me a favour, go take a blood sample and try to get some more information out of him. I need someone who speaks Grey"
Lexie left with Bailey and eventually so did everybody else expect Cristina and Lucy. "Meredith has two groundbreaking clinical trials and I don't have any", Cristina said.
"I know. She sucks"
"Yeah"
Owen, April, Callie and Lucy entered a trauma room where a patient was waiting. "What do we have here?" Callie asked. "You really don't want to know", Owen replied. "Let me get that for you, Dr Torres", April said, grabbing the chart, earning a dirty look from Lucy. "Ok, Randy Shouse, 23, multiple injuries after slamming into a brick wall? How did...?" "Idiot and his idiot buddies built a human slingshot", Owen explained. "I wasn't supposed to hit the wall. I was supposed to fly over the house and land in the pool", Randy said. "But why would you do that?" Lucy asked. "So they could film it, put it on the internet and show the world just how stupid they actually are", Owen said. "Dude!" Randy exclaimed, before groaning in pain. "Did you get that?" They looked over to the window to see a man filming it with a large camera. "Yeah, buddy!" he said, giving him a thumbs up. Lucy rolled her eyes. Owen reached over and closed the blinds. "But why?" April asked. "Because it's hilarious, that's why", Callie said. "Ok, Randy. That must've been some kind of awesome slingshot because it looks like you've dislocated both hips. I'm gonna need to get some X-rays". "I'm on it!" April exclaimed. "Wow, she's perky", Lucy commented. "Better than stupid with a death wish", Owen remarked.
Jackson and Kelly were charting at the nurses station when Mark approached them. Kelly groaned and Jackson gave her a confused look. "Dr Avery, it's about time I had you on my service", Mark said, before turning to Kelly. "You still on my service today?" "I don't really have a choice", she snapped. "I'm interested in plastics, so just try not to talk to me too much".
"Right"
"Did I miss something?" Jackson asked.
"Moving on. Avery, as I said, I'm glad to have you"
"Thank you, Dr Sloan. I gotta say, I really think I have the hands for plastics"
"Forget about your hands. Look at those cheekbones"
"Oh, here we go", Kelly muttered. "Sir?" Jackson asked. "You're genetically blessed", Mark replied. "Don't think I haven't noticed. Speaking from personal experience, I can say that that'll only help you in plastics. That face? Better than a hundred billboards, my man".
"Alright, Dr Sloan, I...really don't feel comfortable being judged or praised based on my looks"
"Oh, poor Mr Green Eyes. Let's not pretend being beautiful's a burden. Now, come on. Mrs Johnson's nose isn't gonna fix itself. How about a little sparkle?"
Jackson forced a smile and they both looked at Kelly. "Oh, you have got to be kidding me", she snapped. "Alright, then", Mark muttered and they walked into Mrs Johnson's room.
Callie, Lucy and April were looking at Randy's scans. "Hip dislocation, fractures of his femur, radius, ribs and metacarpals", April explained in her usual high-pitched tone. "Ok. Should we just go ahead and reduce those hips?" "Kepner, I need you to speak differently", Callie said.
"I'm sorry?"
"Your voice. It's a little... Just speak differently"
Lucy shot April an apologetic look and April tried to smile back while looking very confused. "Like with an accent?" she asked Callie. "Never mind. You know, how about you get me a muffin from the cafeteria?" Callie said. "Get me the sugary-est kind you can find. And some coffee". "Callie, Arizona told me you shouldn't have coffee", Lucy said.
"Well, Arizona's not here. Kepner, coffee"
April started to leave. "Wait. No. Damn it", Callie said, stopping her. "No coffee". "Ok", April replied, going to leave again.
"No, you know what? Yes. Coffee. Please"
"Great"
"Crap! No. No coffee"
"You're sure?"
"No. No coffee. Just... Just go"
"Ok"
April left and Callie sighed. April came running back a few seconds later. "Did you still want the muffin?" she asked. "Yes", Callie replied. April ran off again. "See, was that so hard?" Lucy asked.
"Yes. Yes it was"
Teddy, Arizona, Aurora, Cristina and baby doctor Lucy were in their patient's room. "We know, without a doubt, that this heart is a viable option", Teddy explained. "How? How do we know that this is a good match?" the father asked. "Babies under a year haven't developed an immune system that will reject the new organ", Cristina said. "It makes matching a lot easier". "But we're still four weeks away from the due date", the mother said. "Our OB isn't even in town right now". "I'm covering for Dr Napolitano", Lucy said. "I've got her blessing to go ahead and do the C-section today". "Today?" the father asked in surprise. "That's really...". "Colin, time's an issue here", Alex said from where he stood in the doorway, causing everyone to turn around and look at him. "We need to inform the transplant board as soon as possible if you want this heart. Otherwise it's gonna go to another family". "Ok. You know what?" Arizona said. "There are a lot of doctors in the room right now, so why don't we give the Cookes some space? You can have one of the nurses page us when you've made a decision". The Cookes nodded and the doctors left the room. "What are you doing?" Aurora whispered to Alex when they were in the hall. He ignored her and approached Arizona. "Why didn't you page me?" he asked. "Turnip, Karev?" she questioned, causing Alex to look at Lucy.
"You tattled on me?"
"You are so off this case", Lucy replied.
"Seriously? I have said way worse stuff than that"
"How is that helping?" Aurora asked in disbelief. Arizona and Lucy walked off. "Are you kidding me?" Alex yelled after them, but Arizona just held up her hand to dismiss him. "What did you do?" Aurora smirked. Alex just scoffed and walked off.
Kelly was charting at the nurses station when Mark approached her. "Are you gonna scrub in?" he asked.
"No, I'm good, thanks"
"Kelly..."
"Mark, I'm having a really shitty day and I don't need you around me right now. Just go away"
He sighed and walked away.
Mark and Jackson were operating. "Avery, you want to give the osteotomies a shot?" Mark asked.
"Yes, sir. Mallet, please"
"You live with Kelly now, right? How's she doing these days?"
"Ok, I guess"
"Ok? She hasn't seemed upset?"
"I mean, she kind of cries sometimes. But she seems ok. Now, are you gonna strip the lateral cartilage here?"
"Crying seems fine?"
"She's a girl. Girls cry"
"But you don't know what she's been upset about?"
"Not really any of my business. Yeah... What's next?"
"Next, I finish up here while you go find out what's up with Kelly"
"I'm sorry?"
"She's upset. And you're gonna find out why. You want to be on my service? Then serve. Use that Avery sparkle"
"Dr Sloan..."
"I'm gonna do a couple more rhinoplasties this afternoon. You do me this solid, and I just might be persuaded to let you take the lead on one"
"Ok"
The residents were having lunch in a conference room. Meredith, Cristina and Lucy were looking over Ellis Grey's research. "Holy crap! Your mom was smart", Cristina said. "Truck. Cabin. Spoon", Alex said slowly, his hands on his temples. "Three for three!" Jackson smiled and they high-fived. "Crap! This is so good!" Lucy exclaimed. "Is it better than this?" Alex asked. "'Cause seriously, Mer, you'd have to have died of Alzheimer's to fail this". "I don't even know if I'm doing it yet", Meredith replied. "You have to", Cristina said. "Mother/daughter surgeons cure leading killer". "I can just see it now", Lucy said dreamily.
"I'm so jealous, and I'm doing a heart transplant on a baby that hasn't even been born yet"
"That's still pretty cool", Aurora reassured her. "You keep telling yourself that", Beck smirked. "Will you shut up about that?" Alex said. "You're the one who needs to keep his mouth shut, Cabbage Patch", Cristina said. "Holy shit, what'd you say?" Lucy chuckled. "Nothing, it doesn't matter", Alex replied. "He called the baby a turnip", Cristina said. Lucy fake gasped. "Alexander", she said, pretending to scold him and he rolled his eyes. "Screw this. I've been on this case for a month", he said. "I'm not just gonna sit here". He got up and walked out. "Bye", Lucy said after him. "What's up with him?" Meredith asked her and Lucy shrugged. "Have I ever told you you're a very attentive wife?"
"Ha ha"
The residents returned to their conversations and Jackson turned his attention to Kelly who was sitting silently, flipping through pages of a magazine, not really reading it. "Hey, do you need to talk?" he whispered, and she looked up at him confused. "Definitely not", she whispered back before returning her attention to the magazine. He looked away from her to see Beck staring at him curiously. "What are you doing?" she asked. He just shrugged and continued eating. Lexie entered the room. "Meredith? You have to come see Dad", she said. "Does he want more organs?" Meredith asked.
"He wouldn't say, he was too busy lovin'-up on a tattooed 20-year-old"
Everybody looked up in surprise at this. "I have a surgery to get to", Jackson said awkwardly before leaving the room. "Meredith?" Lexie asked. "I can't deal with my dad", Meredith replied. "I'm too busy with my mom". "Let's go", Cristina said. "I wanna see your dad making out with his tatted up fiancée". "Strangely, I kinda do too", Lucy added. "They're not engaged", Lexie corrected. "Are you sure?" Cristina smirked, making Lexie upset. "Cristina, stop torturing her", Lucy said. "Meredith, please", Lexie begged. "Fine", Meredith said.
Alex waited outside of the OR when Arizona, Aurora and Teddy came out, wheeling the baby in an incubator. "I didn't hear her cry", he said, following her. "Her lungs were junky", Arizona explained. "We had to intubate". "She deteriorated fast", Teddy added. "I'll call for an X-ray and ultrasound", he said. "Already ordered it", Arizona replied.
"What's her pH? You'll need an ABG..."
"Karev, we got it"
"Dr Robbins, she's my patient"
"Not anymore"
Aurora turned around to look at him. "Sorry", she mouthed, before turning back around and following them.
Jackson joined Mark in the scrub room, putting on a mask. "You got something for me, Avery?" Mark asked him.
"I do, actually. She doesn't seem to be paying attention to anything, she's being really quiet and I asked if she wanted to talk and she said no"
Jackson started scrubbing in and Mark turned to look at him. Jackson looked at him and stopped. "I'm not gonna be lead surgeon on a rhinoplasty, am I?" he asked.
"She just needs someone to talk to"
"We are not close. That is not something we do"
"Toffees"
"What?"
"She's a stress eater. You get her eating, you get her talking. She loves toffees"
"That's ridiculous"
"Tomorrow, I'm doing a series of cleft-palate surgeries for the Nightingales Foundation. Pro bono. That sort of thing might look fantastic on the résumé of someone who's in the running for chief resident"
"Might?"
"Get some results...we'll talk"
Cristina, Lucy, Meredith and Lexie watched Thatcher and his new girlfriend through the window. "Get in there", Lexie said. "And do what?" Meredith asked.
"Make it stop!"
"There's no way she's 20", Lucy said. "Yeah, she's gotta be like, 26, 27", Cristina added. "I'm 27", Lexie said. "Oh. Gross", Lucy said. Cristina's pager beeped and she checked it. "Tiny-baby-heart time. Careful when you go in there, Mer", Cristina said. "You tangle with one Hells Angel, you tangle with the whole gang". Cristina walked off and Lucy chuckled. "I better go too", she said. "I'm afraid if I leave Callie alone with Kepner for too long, she'll kill her. Good luck".
Callie and Lucy entered Randy's room where Owen and April already were. Randy's friend was standing at the end of the bed with a camera. "Say it again for the camera", the friend said to Owen. "No, no I don't think I will", Owen replied. "Kepner, talk to the camera so we can get to the OR", Callie said. "In my normal voice, or...?" April asked, causing Lucy to stifle a laugh. Callie glared at April so she just turned to the camera. "Randy has multiple fractures", she explained. "The most pressing problems are his hip dislocations. So we are going to reduce them as soon as possible before they lose blood flow". "Reduce?" Randy asked. "Pop 'em back into place", Lucy said. "Oh, man!" the friend exclaimed. "Hold on, let me get up on this chair for a wide angle". "Yeah. We're not doing it here", Callie said. "He has to be sedated and we have to fix this femur. So we're just gonna do everything in the OR". "Wait, what if we did the hips here, while I'm awake?" Randy asked. "And I can give the thumbs-up to the camera". "You're really not grasping how much this'll hurt", Lucy said.
"It's for the art, Dr Shepherd"
"You know what?" Owen said. "That is a great idea". "Dr Hunt, I think that it's...", Callie began.
"Dr Torres, it is for the art"
Callie caught on and smiled. "Ok. Ok. Ok, yeah", she said, slightly laughing. "Marcus, hop on that chair. Kepner, spot him. All right. Sure". April moved to spot him. "Ok. You guys ready?" Callie asked, as the three doctors took their places. "Ok". Callie and Lucy put their hands on his legs and he screamed in pain. "Please stop! Stop!" he screamed. "Stop! Turn the camera off! Stop! Turn the camera off! Marcus, turn the camera off!" "That totally cheered me up", Callie said. "Art has that effect", Lucy said, chuckling.
Teddy, Cristinal, Arizona and Aurora were operating on the baby. "We've got a strong heartbeat. I think she likes her new heart", Teddy said. "Of course she does. It's delightful", Arizona said and Aurora smiled at her sweetness.
"Alright. Let's take off the clamp, Dr Yang"
"Why is her blood pressure so low?" Aurora asked, looking at the monitor.
"There's a very low arterial pressure wave. Alright, let's check it. Maybe there's a kink in the art line somewhere"
The monitor started beeping. "Damn it! Turn up the flow", Teddy said. "That aorta needs a long diagonal anastomosis". "That'll take at least 45 minutes", Arizona said.
"We don't have that. We've gotta do it in 30, otherwise we're gonna lose her. Scalpel! Clamp it. Start the clock"
"What time do you have, Yang?" Arizona asked. "27 minutes", she replied. "Ok, I'm ready", Teddy said. "Clamp off. Pressure wave's coming back". "Systolic's 57", Aurora said. "Sixty. Sixty-four. Eighty-two". "That's it! Yes!" Arizona exclaimed.
Callie, Owen and Lucy were struggling to reduce the hips. "How did it get to this? How did four seconds of Internet fame become worth breaking yourself into a million pieces?" Owen wondered aloud. "He did it...because he thinks it's funny", Lucy replied.
"That's bull. You just don't yourself at that kind of risk"
"Well, it's his decision", Callie argued.
"His decision. Idiots like this shouldn't be allowed to make any..."
"It is his body, ok! And you don't get to say what he can and can't do with it! He is a person, he gets to make his own damn choices! I am going through the worst caffeine withdrawal of my life because someone thinks that being pregnant means I'm no longer of sound frickin' mind"
"You're pregnant?" April asked, excitedly.
"Shut up! I'm not endangering my baby. I just want one damn cup of coffee!"
"Ok. Kepner, go get Dr Torres a cup of coffee", Lucy said. "Ok", April said, leaving the OR.
"Run!"
"Thank you", Callie said. "Congratulations", Owen said.
"Oh, whatever. Just come around here and help me with this hip! Ready, one, two, three"
Kelly was back charting at the nurses station, not really paying attention to it. Jackson approached her. "Hey", he greeted.
"Hi"
"Candy?"
He passed her a toffee and took it, unwrapping it and popping it into her mouth. "Screw you and your candy", she said. She paused for a second. "You know what bugs me?" she said.
"I..."
"Everybody's making these huge life decisions, and they're not even considering how it'll affect me. How I'll feel. You know, I'm supposed to be such a big part of his-their lives, and yet they're leaving me out of the conversation completely"
He passed her another toffee and she took it with no hesitation. "Starting up new families and springing it on me when it's already a done deal", she continued. "Hey, Kelly, guess what? I'm gonna have a baby and you're just gonna have to be ok with that!"
"Wait a second. Who's having a baby?"
"Mark's having a baby"
He passed her another toffee. Her lower lip trembled and water filled her eyes. "Mark is having a baby, and he didn't even ask my opinion", she said. "He just clobbered me with it. Again. And he's leaving me behind...again. So we're done, you know? We have to be. But I miss him". She put another toffee in her mouth and sighed. "Crap", she said. "I'm so pathetic".
"No, you're not"
"Thanks. For listening"
"Anytime"
They smiled at each other. "Ugh, this is annoying", she said.
"What is?"
"I like you now"
"That's a problem because..."
"Because of Beck"
"Right. Beck and I are just friends, you know?"
"Oh, please. Who are you trying to fool? You guys are obvious. But I won't tell her"
"Thank you"
"But you should"
Jackson entered Mark's OR. "Just in time, Avery. Scrub in and take the lead", Mark said.
"That's ok. I just came by to tell you that you were wrong. Those toffees got me nowhere"
"You're kidding"
"Guess you overestimated that Avery sparkle, huh?"
Jackson started to walk out. "Where are you going? Scrub in", Mark said.
"Really? I... I assumed no results, no surgery"
"I'm not a total bastard. You gave it your best shot. Scrub in"
"Ok"
Jackson and Beck were in the resident's lounge. "So, any plans for tonight?" he asked her.
"Sleep. My plan for tonight is sleep"
"I was gonna hit Joe's before heading home. Want to come?"
"I think I just need to crash"
"Yeah. No, I get that"
"Maybe tomorrow?"
"For sure, yeah"
She smiled at him. She walked over to him and kissed his cheek, lingering for a second. She started to walk out. "Uh, Beck?" he called. She stopped and turned around. "Yeah?" she asked. He was silent for a few moments. "Goodnight", he said and she smiled. She walked off. He sighed and put his head in his hands. "Such an idiot", he muttered.
Lucy entered her and Alex's room where he was already sitting on the bed. She flopped onto the bed and he chuckled. "Tired?" he asked. "Yeah", she replied, her voice muffled by the pillow. He stroked her hair and she popped her head back up. She leaned over and kissed him. "And how was your day?" she asked.
"Alright, but you know what could make it better?"
She laughed and climbed on top of him. He took her face in his hands and smiled at her, putting her hair behind her ears before kissing her.
