Ghosts (or spirits) that are attached to the earth can not be seen. This is simply because they have their very own version of the'Mist'. This version only hides their astral form, and can not conceal changes the ghost, or ghosts, have made to either their haunt or the world outside of it. The way to not be affected by the mist is to know of it. Awareness is it's only weakness. However, with enough time and practice, ghosts can learn to control the mist so that people they wish to see them do. All spirits can do so.

That being said, the only reason a ghost should be out of the underworld is that their soul is tied down by an object, place, or person. This is a rare thing to happen but is not unheard of. Whatever is tying the ghost down shall be followed by the spirit until said thing is either destroyed or killed.

Once someone has gained awareness of the mist, the ghost will be able to interact with that person, such as being able to grab the person instead of going through them.
