*At where the patients are*
Ethan: A 65-year-old female patient. Biscuspid ES.
A bicuspid aortic valve is an aortic valve with only two cusps (or flaps) instead of three. The aortic valve controls the flow of blood from the left ventricle (chamber) to the aorta, the main artery delivering blood to your body.
Ethan: ascending aorta dilatation. AVR. Ascending aorta replacement in progress. Day one of postoperative day. Excubation at ten last night. No signs of complications.
Thunderstorm: Blaze.
Blaze: *looks at Thunderstorm*
Thunderstorm: explain a bicuspid aortic valve.
Lunar: *looks at Blaze*
Blaze: *looks at Thorn then looks down breathing in and out putting his hands in his lab coat pockets* it's a congenital heart defect where an aortic valve only has two leaflets, instead of three.
*this time all the doctors were looking at him*
Blaze: within the aortic valve fusion type, various hemodynamic pictures emerge. In young patients, it will show as AR.
Lunar: *surprised that Blaze knows all of this*
Blaze: in older patients, it'll be AS. Sometimes, there'll be aorta dilatations.😄
Thorn: he had quite the reputation in med school.
Lunar: for what?
Thorn: he knows the material as well as the professors.
Thunderstorm: *looks at Thorn* that doesn't prove anything. *sips coffee*
Thorn: he aced every test he took.
(Author: Thorn is now narrating)
Thorn: He would read obscure research papers and would ask difficult questions that would stump the professors.
*a teacher is seen looking through all of Blaze's research papers with a shocked expression and some of them would fall off the desk as Blaze is standing up talking while all of his classmates looked at him*
Because of him, some professors had to prepare extra hard before a class.
*Thorn is now not narrating*
Thunderstorm: you took a year off from school. You must've kept studying.
Blaze: *nods* ^^
Thunderstorm: Blaze, you're assigned to this patient.
Blaze: what? 0-0
Thunderstorm: *goes to see another patient who is at the same room walking pass Blaze in the process*
Blaze: *has a excuse me?! Look on his face*
Lunar: *looking at the patient's pulse meter*
Thunderstorm: *looks at Blaze again hands behind his back* Blaze. What is the maze OP?
Blaze: *looks at both Thorn and Lunar who are beside him as they also look at him then looks back at Thunderstorm sighing* the maze procedure includes the creation of lines of conduction blocks that block the abnormal impulses that cause atrial fibrillation, enabling restoration of normal sinus rhythm.
Thunderstorm: good. He's your patient, too.
Blaze: *mouth opens with a ehh?? Look on his face*
Thunderstorm: *was about to walk to another patient's bed but then-*
Lunar: but Doctor...
Thunderstorm: *stops then looks at Lunar*
Lunar: this patient is still unstable. I should...
Thunderstorm: Dr. Lunar. Does a Doctor only see stable patients?
Lunar: .....
Thunderstorm: let him. *walks off to another patient*
Blaze: *eye would twitch*
Thunderstorm: *checking that bag that hangs on a stand that has liquid medicine inside it* (Author: idk what it's called sorry QvQ) Blaze.
Blaze: hm?
Thunderstorm: what antibodies do you use for culture-negative infective endocarditis?
Culture-negative endocarditis
is an infection and inflammation of the lining of one or more heart valves, but no endocarditis-causing germs can be found in a blood culture.
Blaze: *smiles then breathes in and out again* I don't know. 😄
Thunderstorm: *looks at Blaze* you don't? *looks at Thorn*
Thorn: *looks at Blaze nodding and then looks at Thunderstorm* we should administer antibiotic medication such as vancomycin,
Blaze: *looks at Thorn smirking nodding indicating he actually knows but pretended he didn't*
Thorn: gentamicin, ampicillin, ceftriaxone, or cefeprime depending on the situation. *smiles*
Thunderstorm: talk to the Infectious Disease Department and proceed.
Thorn: *nods smiling looking at Thunderstorm then suddenly realizes smile fading away looking at Blaze who has a smile looking at Thorn as if hes gonna burst out laughing then looks back at Thunderstorm* me?
Thunderstorm: yes, Blaze says he doesn't know.
Thorn: ..... *looks at Blaze*
Blaze: *trying not to smile so he won't give away that he is just faking it*
Thunderstorm: *looks at Blaze then walks off as the Doctors also follow him*
Thorn: why me? T-T
Blaze: because we're brothers. 😎
Thorn: 😒 *walks away to follow Thunderstorm but then*
Blaze: what about the rest of them?
*the doctors would stop including Thunderstorm then looks at Blaze*
Blaze: *looking at a patient*
Thorn: *looks at Thunderstorm then to Blaze* uhh...those patients are...
Thunderstorm: Thorn.
Thorn: *flinches then looks at Thunderstorm*
Thunderstorm: why are those patients still here?
Thorn: .... w-well, their families are refusing to have them moved....
Thunderstorm: we don't have enough beds.
Thorn: ...... *looks down*
Thunderstorm: have them moved by this week.
Thorn: okay....
Blaze: I see. So a wise and proper doctor gives up on his patient's like them. I learned a huge lesson on my first day.
Thunderstorm: *walks towards Blaze with a serious look on his face* Blaze. There are two types of patients here. Patients that were saved with these hands and patients that can't be healed even after I tried.
And if I can't fix a patient that I operated on, then nobody can. They are such patients.
*talking about the 3 other patients*
You mentioned supporting the livelihoods of staff. A high inflow of patients will result in higher profits. That's a given. Why stick with terminal patients at the expense of other patients?
Blaze: *getting annoyed*
Thunderstorm: I wonder why they're holding on so desperately *looks at the patient's* as if they have a great life ahead of them. *looks back at Blaze* one should know to let go when it's time. Don't you agree.
Blaze: *just stares at Thunderstorm*
Thunderstorm: *was about to walk off again but then*
Blaze: you're so arrogant.
*insert short intense music*
Thunderstorm: ...... *looks at Blaze who is currently talking to the patient* don't you think so? He's so conceited. It's typical of doctors. They're arrogant. More so with him, he's a genius. *inhales* most geniuses have a little madness in them. So ignore what he said. Don't dwell on it. ^^
Thunderstorm: ......
Few minutes later
At another part of the hospital
Ice: *taking care of a female patient who is smiling as Ice smiles aswell* are you eating well?
Random female patient: I ate a little.
Blaze: *the door opens and sees what Ice is doing and smiles leaning on the door*
*Insert a calming yet Romantic music*
Ice: *sees Blaze*
Blaze: *quickly snaps out of it and stops leaning at the door*
Ice: *looking somewhere else confused at who Blaze was looking at then looks back at Blaze*
Blaze: *trying to act normal as if he wasn't gazing at Ice just now and nervously raises his hand with a smile on his face*
Ice: *bruh face but with a smile and blinks once*
(Blice: Author!!!!! (//////////)
Author: :>
Ice: *using his hoodie to hide his blushing face*
Author: admit it~ you guys loved the scene~
Blice: (//////////) ))
Outside
Ice: what is it now? *says with a smile* you're grandpa made you go to med school. Then he told you to become an intern. What does he want from you now.
Blaze: *smirking* the chairman needs to tighten security. *currently holding coffee as Ice is also holding one*
Ice: *chuckles and sips some of his coffee*
Blaze: *smiles* how come an intern like you knows that? He gave me another task. I have to win over the board. *also sips some of his coffee*
Ice: *stops sipping and gasps dramatically but not too dramatic* 😲 really? Must've come as a surprise.
Blaze: it is my destiny as his successor. I have to perform difficult tasks. *bruh face while smiling* I'll be fine as long as I do okay. 😄
Ice: *crosses his arms* then why did you choose the cardiovascular department when it's the toughest?
Blaze: being a heart surgeon is cool. 😎 I don't want doctors to take me as a lightweight later on. 😌
Ice: *scoffs with a smile on his face*
Blaze: 😌
Ice: how about you try your best since you've decided to be a doctor? Change your attitude first. *smiles*
Blaze: hey Frosty.
Ice: hmm?
Blaze: I've lived a sheltered life for 25 years with special privileges. How can I change that now? It's the others that need to change. *bruh face but smiling*
Ice: *sigh*
Blaze: and what's the point? I'm not going to be a doctor for the rest of my life. Theres no need to work my butt off. 😄 *drinks his coffee again*
Ice: *scoffs*
Blaze: *gets a phone call and reaches for his phone located in his lab coat's pocket and answers the call*
Ice: *shakes his head while smiling then looks somewhere else while drinking her coffee*
Blaze: *talking on the phone* yes. Who is this? Yes, Doctor.
Ice: *listening still looking the other way*
Blaze: an operation? But why would I? *acting mode on* I'd really like to be there but I'm not feeling well right now. 🤧 *the call hangs up making Blaze blink 3 times* he hung up on me.
Ice: *looks at Blaze and shakes his head turning around about to go back inside but then*
Blaze: why do I need to observe Dr. Hali performing surgery? *talking to himself*
Ice: *stops then looks back at Blaze*
At the operating room
The door slides open revealing Thunderstorm in his surgical outfit
Thunderstorm: *goes inside, sees other people there and goes to his spot wiping his hands using a tissue*
Ice: *is one of the observers in the observation room but this time is wearing glasses*
Thunderstorm: *puts on that coat that operators put on when operating on a patient as one of the doctors there helps Thunderstorm put on his gloves*
Ice: *countinues to observe*
Thunderstorm: *notices the interns* why are there so many interns?
Lunar: watching you operate is a great learning experience for them. And this is a rare opportunity. Plus, they wouldn't want to miss this complex procedure.
Thunderstorm: *looks at the interns who are observing* what's there to see? It's just another surgery.
The door slides open again
Blaze: *comes in*
Ice: *notices*
Thunderstorm: *also notices* you punk. You're late on your first day. Wear a surgical gown(Author: thank goodness in the actual series it said what it's called cause I literally forgot QvQ) and come back.
Blaze: *looks at Thunderstorm then looks at Ice who is one of the observers*
Ice: *nods*
Blaze: *smiles then wears a surgical gown*
Thunderstorm: stand close behind me.
Blaze: *does as told*
Thunderstorm: *closes his fist then opens them repeatedly the operation will start. *twists his wrist*
Lunar: this is the fifth operation for the patient.
Thunderstorm: *stops twisting his wrist*
Random Surgeon: there could be fused tissue, so we have to carefully open up the chest.
Thunderstorm: *breathes in and out calming himself down*
Blaze: *watches*
Thunderstorm: shall we begin? Scalpel.
*A surgeon would give him a Scalpel*
Thunderstorm: *uses it to open the chest slowly and carefully*
Blaze: *countinues watching*
*A bit of blood would come out*
*A doctor would notice and would suck the blood up using a special suction tool*
Thunderstorm: bovie. *a doctor would hand Thunderstorm a bovie and uses it and looks at the pulse meter then goes back to looking at where he is operating*
(Author: Idk how to put the rest so uhhh ima just skip sorry QvQ)
Thunderstorm: done. Switch places.
Lunar and Thunderstorm would switch places
Lunar: army navy retractor. *a surgeon would give him the item and uses it as Ice just countinues to observe same goes for the other interns* hand me the retractor. *a surgeon hands it over and uses it* hold it. *a surgeon does as told* all right. RA first.
Blaze: *just countinues watching*
Meanwhile at Administrative Director Rettaka's office
Rettaka: *gets a phone call and answers it* yes? This is Rettaka. Then let's say tonight. *looks at the doctors who are currently operating through the tv and scoffs* you don't need to worry about that~
Back to the surgeons
Random doctor: the main procedure is over. We'll perform transesophageal echocardiography. There's no aortic valve para valvular leak. (Author: to lazy to search up what it means sorry QvQ) no leakage around the stitches.
Thunderstorm: *listening and so is Blaze*
Random Surgeon: the average systolic pressure stayed below 1.5 m/s.
Ice: *also listening*
Random Surgeon: No blood vessels narrowed. So the operation is a success.
Blaze: *nods*
*All the interns who watched the surgery in the observation room would clap except Ice*
Thunderstorm: *looks at Blaze* Blaze.
Blaze: *looks at Thunderstorm*
Thunderstorm: what is a bioprosthetic aortic valve?
