Here's the format I'd encourage you to follow when filling the form.

Username

The Title of the book

Subtitle (If there is any)

Synopsis of the story (So I could grab the gist of it)

Any particular images/colours which you would like me to use

Character face claims. (If you do not want a face claim, include this too)

Your name (So I could include it in the cover as the author. It'd be better if your name doesn't consist of numbers or symbols, such as an underscore or other)

If you've failed to include these details in your form, please note that I would disregard them :) I need your cooperation.
