Hi. I'm sorry to everyone who has been dedicated to this fic over the past year, but this is the end. Whilst I enjoyed writing this immensely, I have lost all motivation to write anymore. Instead of writing the remaining few chapters of the story, I thought I would just tell you the plot so that you don't miss out on a proper ending.

The diary Draco kept was forgotten by him, but found by Blaze at the beginning of sixth year. He held onto it, knowing that Draco would want it for memories later on. Unfortunately, this did not span out as he had hoped. During the trio's hunt for horcruxes, Voldemort tortured Blaze by removing his bones and forcing him to regrow them. As this process took place, Blaze's bones were replaced almost entirely by Voldemort's bones, essentially creating another horcrux.
Draco's diary has been charmed so that his whereabouts is always known by Blaze. He then gives this information to Lucious (who escaped from Azkaban with Blaze/Voldemort's help. Blaze gave his wand to Lucious in order for him to use more powerful spells to escape). Luscious does not want to harm his son in any way, but he is loyal to Voldemort and wants to kill Harry.
Blaze and Lucious turn up at Hogwarts and attempt to kill Harry. Harry and Draco escape to the Quidditch pitch and hide from Blaze and Lucious under the seats where spectators watch the game. Draco contacts Narcissa via a small magical notebook that sends a message to Narcissa when he writes in it (basically wizard texting). He simply writes "come to Hogwarts" so she does not realize they are in danger, and brings Teddy along. 
They all duel on the Quidditch pitch. Blaze/Voldemort attempts to 'kill the spare' but Narcissa saves Teddy, sacrificing herself. He is saved by a mother's love (technically aunt not mother but anyway). Lucious uses himself as a shield to block Voldemort's harmful curses aimed towards Draco, who is trying to protect Harry whilst grieving his mother.
Harry manages to remove Blaze's bones, therefore removing Voldemort from his and killing him for good. Blaze is rushed to Madam Pomfrey and Lucious falls unconscious.
Aurors arrive (including Ron) to take Lucious away. They heal him enough to be able to give a final goodbye to his wife and he discovers that she is alive, but only just. The curse used by Voldemort was not the killing curse as he was too weak to use it, but was strong enough to kill a child such as Teddy. Narcissa was able to survive the curse. She is then rushed to St Mungo's.
Lucious is taken away. There is no mention of Draco coming out to his father, and it is decided that he most likely never received the letter telling him so.
Harry picks up Teddy, who until this point has been sitting on the Quidditch pitch, unharmed and seemingly unbothered by the events that have just occurred. Draco, Harry, and Teddy then go and sit by the lake and have a moment of peace that they know will not last long, and wait for the aurors to return and question them.

So that's it! The whole peaceful moment thing kind of comes full circle, which I like. If you don't know what I'm talking about them read the first few lines of the first chapter.
Again, thank you so much to each and every person that has stuck with me. I am sorry that I am unable to give you a fully written ending. My brain, which barely works at the best of times, will just simply not allow it.
