He does have lungs; they'd be behind his chest plates. Wookieepedia has the answer: Grievous's armor and synthskin gut-sack protected his remaining vital organs, but his organic lungs were irritated by their implants)

I'm assuming since the 2003 Clone Wars 2d animated miniseries is not considered canon, it's viewed as "Legends". Meaning it's a fabled campfire telling of true events at this point.
I don't believe canon The Clone Wars points anything about it, but one main canonical reason I've seen many times is that his cough was initially thought to be put in when Lucas came into work with bronchitis. His cough caused inspiration.
The cough is used mainly to convey to the audience that he is not merely a droid; he's organic. Droids have no need to cough.
In universe answer: after a horrific accident, the alien warrior had to be provided a new body, Dooku scooped him up and gave him one. His organic bits are having trouble bonding with/accepting the foreign cybernetic materials. His body wants to reject the foreign contaminants, possibly he has some sort of infection.
Although Windu epically crushing Grievous during his escape sounds more fun/cool/exciting; i just don't think it's canon anymore. And we're left with a much more simple/realistic reason lol. Unless there's new reasoning I'm unaware of.

He's an organic creature being kept alive by machines. His body is rejecting it, and it's being forced to work harder. In a way, it also mirrors Darth Vader's ominous breathing.
In a way he is the beta version of Vader. they say the coughing was excess fluid in his damaged lungs.Ironically his cough sound is recorded by George Lucas' himself.!
