86. These Times That Bind

I was kinda glad that nothing bad has happened with the patient from my last entry. When I went to work yesterday, I said that Hahn's name had completely just vanished. Straight into fine air. When I got to work I saw a familiar face with Mer and Cristina. "Sadie, since when did you get here?" I asked while walking over to her.

"You know her as well?" Mer asked with a smile.

"Yeah, we used to go to primary school together." Sadie asked while engulfing me into a hug.

"Oh, great." Cristina said before walking off.

"How do you two know each other?" I asked as we started walking together.

"We went hiking around Europe together. Along with a few other things." Mer answered. Small world we live in. I went to the locker room and changed before getting paged to ambulance bay. I saw Callie and we walked outside together.

"Hey, what do we got?" Callie asked both Hunt and Cristina.

"Crush injuries." Cristina said to us, this was gonna be fun.

"A couple of minutes out." Hunt said, finally I get to work with bones again.

"Hey, what do you know about Hahn?" Cristina asked Callie while she was tying up the back of the gown up.

"She's gone." Callie answered shortly.

"Yeah, but why?" Cristina asked.

"I don't know." Callie answered, it was best not to ask.

"You two are friends." Cristina added.

"I don't wanna talk about it. I want blood and guts and mangled bones. I want a guy so badly smashed up, it takes all day to fix him. I wanna rock a surgery. I don't wanna talk about Hahn." Callie said as the ambulance was getting closer.

"Ok, ok." Cristina said before the ambulance stopped in front of us.

"Unidentified man with prolonged extrication time. We had to fish him out of a working garbage truck. Multiple crush injuries, open fractures, GCS three, and we couldn't unhook him in the field." The paramedic informed us. I was confused a little.

"Unhook him?" Owen asked her.

"What do you mean by that?" I asked her confusedly.

"His femur's broken in half and is puncturing his torso. He's impaled." She told us, they got him out the back and I looked at him. Oh god, we're the only things that came to mind.

"Impaled on himself?" Cristina asked.

"Human pretzel." Callie said with a little smirk. "That'll do." She added before we all went inside. We got him to a trauma room and paged Mer.

"Wow." I heard her say as she entered the room.

"I know right." I said while looking at her.

"Check his popliteal pulse." Owen said to us.

"I'm getting nothing." Cristina said to him.

"That's because his knew is over here." Callie said to her, this was gonna be hard.

"You think he was sleeping in the dumpster and they started crushing the trash?" Cristina asked us, I mean it was possible.

"Maybe." I said to her.

"That's a bad day." Mer said, I saw someone else enter the room.

"Hey, I'm here. What did I miss?" Sadie asked while walking in.

"Hey, it's your first day, why don't you just stand back and shut up. Breath sounds are clear and equal. Both sides. Unbelievable." Cristina said to us, I looked between Sadie and Mer. That wasn't fair with what Cristina just said. Sadie was an intern that was here to learn.

"So, no punctured lung. We need a portable X-ray and an ultrasound." Owen said before Mark and Derek appeared at the day.

"Someone page us." Mark said, we honestly need all hands on deck near enough.

"I did." Owen told them both.

"You did? That's a first." Derek said to him, looks like today is filled with firsts.

"So is this. I've seen a lot of trauma guys cut in half, amputations, eviscerations, but I've never seen anything like this. I wondered if you could help." Owen said to both Derek and Owen. They walked over and they looked at our John Doe. We got took him to have multiple scans and looked at all the results that had come in.

"His femur's in pieces. Except for the part that's stuck in his torso. There's no way I'm saving that leg." Callie said to us, I knew that from the second he arrived.

"Looks like the impaled piece is lodged under his clavicle." Mark said while looking at them.

"C-spine looks clear. Without a CT, it's hard to be sure." Derek said, we'd have to go in blind which wouldn't be good.

"Why can't we get a CT scan?" Sadie asked. Has she seen the state of this man?

"The way he's impaled, it's hard to get a clear reading." Derek said before walking away.

"You know, there is such a thing as a stupid question, stand over there." Cristina said to her, why was she being so harsh.

"Guys, we have a very limited window here, ok? With vitals like these, in a few hours- -" Callie started before Mer cut her off.

"His body won't be able to withstand surgery at all." Mer said to us.

"It's a good thing that he's unconscious." I said to them all.

"We can't do anything till we dislodge the femur from his clavicle." Mark said to us.

"Ok, then let's unimpale him. Yang, Grey, Y/l/n, new kid, hold his shoulders. Brace him hard." Owen said to us, we all grabbed a shoulder, it was two on one.

"I'll hold his C-spine." Derek said to us.

"I got his torso." Mark said to us.

"I need to guide the posterior hip dislocation." Callie said to us.

"Second that leg is out be ready to assert pressure, stop the bleeding. Got it?" Owen said to us, I nodded.

"Better than working on corpses, huh?" Mer asked us, it truly was.

"On the count of three. One...two...three!" Owen said to us before him, Callie and a nurse helped put his leg right.

"Femur's really wedged in." Cristina said to them as it wasn't moving.

"Push directly on the clavicle while I pull." Callie said to Mark. "Ok, almost there. Almost there, steady. Just a little bit further. Sloan, I need you to rotate his knew to my right- -" Callie said before the guy woke up and started screaming making me jump out of my skin and making Sadie scream.

"Push ten of morphine now!" Owen ordered.

"Stop! Stop!" The man shouted, we couldn't stop seeming as we were almost done. He was shouting in pain.

"Corpses don't do that." Sadie said to us, we were trying to stop the bleeding but the guy just kept squirming and screaming.

"I need you to calm down, sir." Owen said to him, that was going to be impossible.

"What are you doing to me?" The man shouted at us.

"Sir, can you tell us your name?" Derek asked him in a calming tone.

"Get out of my face and take your hands off! You're hurting me!" He shouted at us, we were helping.

"Do as he says. Hands off, everybody." Owen said to us, we all followed instructions and took our hands off. "Hands off!" He repeated, Derek then took his hands off along with Callie. "Thank you. What's your name?" Owen asked him.

"Timothy Miller." He answered.

"Mr. Miller, we need to get you into surgery." Callie said to him calmly.

"I'm gonna be able to walk, right? I gotta be able to walk." Mr. Miller said to us, the chances of that were unlikely.

"Right now, we're just trying to keep you alive." Derek said to him, we went back to Mr. Miller to tell him the news.

"I won't live if I can't walk." Mr. Miller told us, in some way I felt bad for this man.

"We'll do our best to save the leg." Callie said to him, I feel like we won't be able to save it.

"I'm homeless. Walking is how I survive, it's how I get from one day to the next. Marion Street's safe from 2:00 to 6:0l a.m., and then I go over to Hadley Bridge and I hang there till 6:30 until traffic hits. And then I go over to Belltown, and then I hang out over at the waterfront. And then the waterfront, if it's trouble, then I go back up to 4th Street. I'm homeless. I can't not walk! I...what am I gonna do? What am I gonna do?" He said to us, I truly did feel bad for this man. We needed to try all we could to make sure that he could walk again.

"Your injuries are critical. If we don't get you to surgery- -" Mark started before Mr. Miller stopped him.

"Will your surgery make me walk? Huh?" Mr. Miller asked but we all stayed silent. "Then I don't want it!" He shouted.

"Ok, all right. You heard the man. The surgery's off." Owen said while taking his gloves off. I get where he's coming from but we needed to do something. Owen walked out, a short time later Derek and Mark followed him. When the rest of us got to them, Owen had walked off.

"Dr. Torres, please tell me you can figure out some way to build this man some legs." Derek said to her, she didn't say anything. Me, Sadie, Cristina, Mer and Mark followed her and watched as she was trying to figure something out.

"The right femur's toast. So is the hip and half the pelvis, but if I put some pins here, and maybe a plate here...I mean, there are some viable bone fragments, we are not starting from nothing. What do you think?" Callie asked me and Mark.

"I think it's wild." Sadie said to us.

"All right, she wasn't talking to you." Cristina said to her.

"It is only her first day. Could you please not be so- -" I started saying to Cristina before I was cut off.

"You know, she's an intern. She needs to learn her place." Cristina said to us, she wasn't like this when she was intern.

"I think it's a lot of hardware. What the guy needs is a whole new leg." Mark said to her.

"We could attempt to make a new leg but it would be a lot of work." I said to her getting a nod from Mark.

"Maybe I can do that. Yeah, I mean, it's kind of a puzzle. I just need to figure out what equipment I need and how to pull it off." Callie said with a smile creeping on her face.

"Like Lincoln Logs." Sadie said from behind us.

"It's a little more high-tech than that." Cristina said, it wasn't actually.

"It's exactly like Lincoln Logs. Okay, I need you guys to grab every piece of titanium we've got in the hospital." Callie said to us, it was willing to take on that challenge.

"Ok." Cristina said to her. We then left the run in hunt for titanium. I managed to find some and put it onto a trolley. I then took it back to Callie and Mark. It stayed with Mark and Callie and helped with what we they were doing. When Callie and Mark left for a procedure I was asked to stay to try and figure out a plan. When it was time we all scrubbed in after Mr. Miller had been prepped and started the reconstruction.

"I've never seen a reconstruction like this." Cristina said to us, no one had.

"Well, I've never done one like this. Here's hoping it works. All right, I'm ready for the rod. How are you guys doing up there?" Callie asked the others.

"There's a lot of hemorrhaging bleeding through the packing." Owen said to us, this wasn't sounding good.

"Try the fibrin sealant." Derek said to him.

"Good call, thank you, Dr. Shepherd." Owen said to him.

"How you doing on that lac, Little Grey?" Mark asked, who was Little Grey?

"Good, moving to the next lac." Lexie said, since when did Mark come up with that nickname.

"Finished already? Let me see. Wow. Pulley stitches. Nicely done." Mark said while looking at what she'd done.

"Thank you, Dr. Sloan." Lexie said before we continued on with the rest of the procedure.

"Just thought I'd stop in to see how everything's going." Webber said while checking in on us.

"Well, I'm still several hours away, but I may actually pull this off. Rebuilding a man's skeleton from scratch." Callie said, I could tell that she was smiling.

'And we just found out that Dr. Grey here can out-pulley stitch Shepherd." Mark said making Lexie and Derek looked at each other.

"Oh, I wouldn't go so far as to say that." Lexie said to him.

"Excellent job, Dr. Grey. And excellent job of teaching, Dr. Yang." Webber said to them both.

"Sir?" Cristina said confusedly.

"Taking the time to teach your interns, and they're retaining that knowledge. I haven't made my decision yet on the solo surgery, but you keep teaching like that and you're my one to watch." Webber said to her before walking out. But she hasn't taught any interns at all.

"Thank you sir." Cristina said to him. Mr. Miller started crashing.

"He's crashing, V-fib." Mer said to us.

"Start CPR. Push one of epi and charge the paddles to 360." Owen ordered. I charged the paddles and took them.

"Right away. Clear." I said before shocking him. Nothing worked, he was gone.

"Time of death, 19:22." Owen said to us. I took my mask of and looked at his leg.

"We built his bones." Callie said while looking at me.

"You tried. We all tried." Owen said to us.

"No, we didn't try, we did it. We did our part, we built his bones. We built his bones from scratch, and you, all of you, you were supposed to keep him alive. That is all you people had to do. You just had to...keep his body breathing. You just had to keep him safe and breathing and living until we made his bones. We made...we made his bones. Until we made his bones. We made his bones.we made his bones." Callie said while crying.

"It's ok Dr. Torres." I said to her.

"Why don't you guys go ahead and we'll help Dr. Torres clean up." Cristina said to us while Callie was crying. This was all about Hahn leaving her.

"Go call the morgue." I said to them.

"We've got it. Go." Mark said to them.

"Everyone, clear out." Owen said to everyone. Me and Mark stayed in the OR with Callie and pulled her into a hug. When we finished up Mark went to meet up with Derek whilst I stayed with Callie. We only separated when I went to get changed to head home. When I got home I poured myself a very large glass of wine, I needed it after today. I decided that the best thing that I could do for myself is to read a few case studies and articles.
