159. This Magic Moment

Today was a different day for all of us. We were called into the OR. We were splitting up into two teams today. "Practice, practice, practice, until it is perfect, people. This is a long, complex procedure with many parts. Therefore many opportunities for error. So we're gonna go over this again. Team leaders, let's get in place." Owen said and that's when we started to split up.

"All right, green team over here, please." Derek said and I moved over into that side with Callie.

"Let's go red." Arizona said from the other side.

"We've offered this family every pro bono dollar, every surgical hand, every resource this hospital has. So I'm asking you for every resource that you have. Be sharp, be present, be focused. Dr. Robbins has been caring these children since the day they were born. They will each get a new life today. Our actions decide what kind of lives they will be." Owen said and it took the cover off the table that's when we saw our practice dummies. "Let's begin." He said and that's when we started our practice time. We didn't have time for errors.

"Flaps are inset and viable." Mark said to us and I looked at him. This was going to be a long procedure.

"We are looking at at least 18 hours, so come and go if you need to, hydrate, eat, but be here at least 30 minutes before your procedures." Owen said to us and that sounded good to me. Who knew what could come up.

"All right. Next up, general surgery. First we need to establish each baby's blood supply to the shared colon." Arizona said to us and I looked at her.

"Then we separate the colon, making the first incision- -" The person doing the general on our side said to her.

"I will make the first incision. We'll separate the colon." Arizona said to him, I had a feeling this wasn't going to go well.

"Ok, and now I'm checking the displaced kidneys- -" the other surgeon said but Arizona cut him off again.

"To make sure they're intact and functioning." Arizona said to him, I think he was getting to that.

"Uh, that's implied." He said to her.

"I don't care. Say it." Arizona said. Derek and I both looked at Callie.

"Checking the kidneys to make sure they're intact and functioning." He said to Arizona. I hope that gave her peace of mind. I was just watching them both bicker.

"You forgot to cauterize. You want this baby to bleed out?" Arizona asked him, this was going to be a long surgery.

"Do I? No, I- -" He answered back to her. Jackson and I shared a look with each other.

"Yeah, I should think not." She said to him. This was something that would make me lose my confidence entirely. "It's time to flip." Arizona said to him.

"Karev, you're up." Derek said and we let him get in. We all moved around.

"Ready?" She asked him and I looked at Alex. He was incredibly nervous.

"Yeah." He said and that's when they started to practice flip.

"Karev, you rushed it. You're like two steps ahead of me." Arizona said and I looked at Callie. Arizona was way too tense.

"No, you over me. You're over mine, everything's fine. Mine over yours, babies in the morgue. That's how it works." Alex said and that was a bit of morbid poem.

"That's your pneumonic device?" Arizona asked him, I'm all fairness it did work.

"Great. Now Brandi's wounds are exposed, and little Andi's gone without oxygen for a whole minute." Owen said to us, we really needed to get this all under control.

"Ok, I- -Can we start over?" Arizona asked, we had no starting over in the real thing why have one in the practice.

"No, there's no time. Let's just finish this practice and try the flip again. Next." Owen said to us and I looked at Callie.

"Neuro." Derek shouted.

"Ortho." Callie shouted.

"Ok, We will access and separate the conjoined spinal canal." Derek said to us and this was the part where things could get more complicated.

"Separate the bony elements of the spine." Callie said to us all and I saw Bailey walk in.

"Meredith Grey. Ok, come with me. OR 2. Right away." Bailey said before walking out. Mer then went after her.

"Spinal separation complete." Callie said after me managed to do it. I heard Cristina's phone go. She left shortly after.

"Ok, I am now prepared to separate them completely." Derek said to us and this was going to be the life changing part.

"Wait. Hold on. What about complications? I mean, what if there's a double hemivertebra or a tethered cord?" Arizona asked and there was the questioning for us.

"We will cross that bridge when we come to it." Derek said to her and Owen looked at us.

"We're done here." Owen said to her, I was glad about that.

"I-I'm sorry. What if we're standing in that bridge right now?" Arizona asked and Owen wasn't impressed.

"Dr. Robbins, I said- -" Owen started but was cut off.

"No, no, no. We haven't even separated them yet. We haven't gone through the post-separation surgeries." Arizona said to us, but we wouldn't need to think about those surgeries yet.

"Dr. Robbins, stop." Owen said to her but she didn't.

"There's so much- -" Arizona started and Owen cut her off. Again.

"We're done practicing. We have practiced our parts not only here in this OR, but in the shower, on your drive to work. We are ready. We know what to do. Now we just have to do it." Owen said to us and I was nervous to do this. When they brought the babies in. They were the sweetest looking. "Ladies and gentlemen, I present to you Andi and Brandi Edwards. Let's give them new lives, shall we? It's showtime." Owen said and it was time to begin. I then got a phone call.

"I've gotta take this but I'll be back for our part." I said to Callie and Owen. It was the immigration office. I was nervous. I then found somewhere quiet.

"Hello, Dr. Y/n Y/l/n speaking." I said as I sat down on a chair in the waiting area.

"Hi, this is Kerry Shaw from the immigration office. I just wanted to know about your status for applying for a green card. You don't have a lot of time left. 6 months at most." She said to me and I looked at the nurses desk.

"I'm working on it. I have medical boards coming up so that's been taking over my mind but trust me it will be coming soon." I said to her and I was then watching the clock.

"I can see how that will be a stressful time for you. I can see no reason as to why you wouldn't be able to get a green card. You've saved a lot of lives while you've been here and your working on things that could change the future of medical history. You've got a strong case." She said to me. I kept talking with her. We were discussing what to do and what I should say. I then went back into the OR. I scrubbed in and that's when I had to wait, we then had our time to do the spinal cord.

"Damn it. Spinal dermoid cyst. It's all tangled up in the spinal root." Derek said to us. It was a clean and easy thing either.

"Yeah, there's a lot of scarring in there. I don't see a clean approach." Callie said to us and I didn't know what we could do.

"Well, no, well, you have to take it out because the cyst could become malignant." Arizona said to us and I understood.

"We'll do that don't worry." I said to her.

"Yes, we're aware. Thank you." Callie said to her wife. I could tell she was getting annoyed.

"Maybe if I can come in from an angle...like this, but then we risk the contents spilling out-  -" Derek started before he was cut off by Arizona.

"Which could cause a chemical meningitis." Arizona said to us.

"Dr. Robbins, we've got this. Ok, what if we stretch the nerve root here?" Callie asked, we had to think of the possibilities but we didn't have time for that.

"Well, that might paralyze Brandi." Arizona said to us, these babies not have a perfect life after this. This was a risky surgery.

"Yes, Brandi might end up paralyzed. Yeah, no one understands that better than we do, which is why we will be making this decision, ok? Us. Not you. Us. You all right with that?" Callie asked her wife. She was finally getting through.

"And trust me, we won't decide on anything if we didn't think there was another option." I said to her, this was the only thing that could be the potential outcome.

"Just make sure- -"Arizona started and Callie interrupted her by clearing her throat. "I- -Yes, I'm ok with that." Arizona said with a nod. She wasn't really.

"No more talking from anyone on this OR. We need complete silence from now on." Derek said and that's how it went. And soon we were going to separate the girls. "Ok, we're ready for the final cut." Derek said and he made it. Just like that. The twins were separated.

"All right. Separation complete." Callie said and the clapping then started up.

"Guys, too early." I said to them all but they didn't listen.

"No, no, no, no, no. Not yet, not yet. We still need to check on the motor function. Let's do...two milliamps of simulation on Andi first, please." Derek said and we saw a foot move. That was good.

"All right. Let's try Brandi now." Callie said to us all.

"Two milliamps of stimulation." Derek said and nothing. We all looked at each other. "Let's go up to three milliamps, please." Derek said and still nothing.

"Don't worry us like this Brandi." I said to her. Even though she didn't have control of it herself. I was hoping she'd still move.

"Come on, Brandi. Just give us some kind of movement. Anything." Callie said and nothing. Derek even tickled her for but still nothing.

"All right. Let's go to four milliamps of stimulation." Derek said and I was hoping we'd get something. We did. "Hey, there you go." Derek said and everyone clapped.

"Great work, everyone. One step at a time, by the numbers like we practiced. Let's move." Owen said and just like that it was time to get the other stretcher in. Someone then opened the door.

"Dr. Y/l/n, there's someone in the pit that's asking for you. He won't leave." A nurse said and k looked at her.

"Did you get a name?" I asked her and she looked at me.

"He said his name was Canda." She said and I looked at Callie. I didn't want to leave this.

"Go, we got this." She said to me and I looked at her with a nod then left. I then went up to the pit.

"There's the doctor of the hour. Now I just need a few stitches." He said to me and I looked at him and shook my head.

"I really wish you let someone else do this. I was doing a groundbreaking surgery and now, I'm having to do first year work. All because another ortho surgeon wants to be worked on by another ortho surgeon." I said to him and I looked at him. He technically was still a patient. Even if he was a pain in my ass. "I'm sorry, I shouldn't have shouted at you like that." I said to him and he looked at me.

"It's ok, I wish I let someone else do the stitches." He said to me and I started to stitch him up. I worked in silence. He didn't even talk to me. And once I'd gone back to the OR. It was too late for me to even do anything anymore. I went to grab a coffee and that's when I saw Jackson.

"You ok?" He asked me and I looked at him.

"I need to apply for a green card. I haven't applied for one yet." I said to him and he looked at me. Taking my hand.

"When we get home. We're sorting it out." He said to me and I smiled. We then went to go and see the babies. God how I wanted one. But that would be coming soon. Hopefully.
