149. Free Falling

I was heading back to work, the place where I had the worst day of my life a couple days ago. When I got there I saw Jackson. Me and him still weren't living together. "Good morning." I said to him as I was heading off to my breakfast. It's crazy to think that this was my final year.

"Good morning, how are you feeling? Final year." He said to me as we started to load our plates up.

"I feel like I've ever experienced more trauma here then anywhere else in my life." I said to him and he nodded in agreement.

"The only thing to complete it would be something else happening to us before the hospital goes normal." He said to me and he was probably right. Maybe one more experience and this would all be over. But I didn't want that happening.

"Good morning, and welcome to your final year of residency. Um...This breakfast is the one time when all the fifth years and the attending will be in the same room. So- -so use your time wisely, residents. Find a mentor, ask questions, get advice on your futures. And, um, I see you're already chatting away, so that's- -that's good. Just carry on." I think I heard April said but I wasn't really lusenyinh.

"Do you know where you'll go to after boards?" I asked him and he looked at me. Shaking his head.

"I haven't thought much about it yet. Have you?" He asked me and he looked at me. I nodded at him.

"I was thinking California or New York. I don't which one I'd rather do. At this rate I'll probably just stay here." I said to him and he looked at me.

"Well, wherever you go. I'll follow." He said to me making me smile. We then kissed as we sat down at our table. I then saw Callie.

"Give me a minute." I said to him and he nodded as I went over to talk to him.

"I was hoping that you'd come and see me- -" Callie said before she was interrupted by April.

"Oh! Look, here's the Chief! Chief, is there anything you'd like to say as we embark on our final year of residency?" April asked him and this was where I was hoping for some thing motivational. We all looked at him awaiting his words.

"You all know what you're doing. Just dont kill anyone." He said to us all. Well that was good advice. "Uh, Meredith, can I see you in my office?" He added and Callie turned back to me.

"Back to what I was about to say. I've been getting a lot of applications for the ortho fellowship but I haven't seen yours." She said to me and I looked at her.

"That's because I've been thinking of what to put. You know about me already, so what's new that I can do." I said to her and we began to walk.

"Just write the most spectacular thing we've done. Besides you know I'd make sure you got the placement." She said to me before we were paged. We had a trauma coming in. We went to the ambulance bay and waited.

"We're going to the scene of an o site amputation woman's trapped under her car. You guys are gonna get swamped with bodies. Kepner, you're in charge till I get back. Y/l/n, your coming with Callie and I." Owen said and I immediately followed them into the ambulance. I guess Callie wanted me to come.

"Yes, sir. Have fun." April said, who the hell says something sick like that. Someone was losing a limb.

"At an amputation?" Callie asked looking back.

"That's a bit dark." I said looking back at her.

"Uh, I-I meant good luck." She said and just like that we were off. Once we arrived in scene, I took it in. A lot of people were injured.

"Dr. Torres?" The incident commander asked as we got out. On the journey here I'd taken my yellow gown off.

"Yeah, hi." Callie said back to her.

"Dr. Y/l/n?" She asked.

"That's me. I wish we were meeting under different circumstances though." I said to her.

"Dr. Hunt? Karen Lorenz, in charge of search and rescue. We're still pulling victims out of the debris, but we have a couple, Danny and Susannah Wilson, stuck down there. He has a head wound and his ankle's messed up, but I don't see any other obvious injuries. We could potentially bring him up, but he won't leave without his wife, and her leg is stuck under the car." Karen explained to us, and with given how big this whole crater looked. There would be no way to pull the car up. Especially since her injuries could be worse than we think. This hole was still growing. Make our situation dangerous. Let alone the fact that water was starting to flood it as well. We then started walking over to where the computer was set up. Displaying what it was looking like down there. "It was a water main break on 3rd. Probably been going on for weeks, and it just gave way. Thanks. We've got a vitals monitor and a camera, so we have a visual." Karen said before Callie put a earpiece on. We were all given one so we could communicate.

"Danny, can you hear me?" She said over the earpiece.

"She's-she's not talking anymore. You gotta-you gotta get down here." Danny said to us and he sounded desperate. Looking at the car placement it would be an above the knee amputation.

"Danny, Danny, I'm Dr. Hunt." Owen said to him.

"We need help." Danny said to us.

"Can you read out for me the top three numbers on the monitor?" Owen asked him and I was hoping that these were good numbers.

"87. 63. 122." He said to us, this wasn't good at all.

"BP's 87 over 63. She's shocky. How do we get down to her?" Callie asked, it had to be soon.

"You don't. It's too unstable. The whole thing would cave in if we sent you down there. By the time we get it showed up enough to get someone down there, she'll be dead for sure." Karen said and wasn't she a world of positivity.

"I thought that's why we're here, for the amputation." Callie said and that's when Karen went silent for a second.

"You're gonna talk him through it." Karen said to us and I looked at her.

"Is he even in the right mind state for that?" I asked her, he could easily mess this up and cause more damage then needed. She could bleed out before she even comes back up.

"You want him to cut off his wife's leg?" Owen asked her and that's when Danny came back over the monitor.

"Hey, you guys, I think she's dying. Get down here! Please hurry. Hurry!" He shouted at us. I wish I could get down there. There would be an easier way to for us. I knew there would be.

"Danny, you've got a kit. Now, they're sending down a second harness. The doctors are gonna talk you through this, ok?" Karen asked him, and there was no way he'd be capable to do this.

"Ok." Danny said to us. That's when I saw a familiar face walk over. Alex. We know he was coming seeming well April had called.

"What can I do?" He asked, go back.

"Kepner called. She wants you back at the hospital." Callie said to him and he wasn't going to do that.

"I can't go back there." Alex said to us and I looked at him.

"You need to." I said to him. He needed to face the music.

"Yeah, it sucks that everyone hates you. But you're not hiding out here. Go." Callie said to Alex and he started to walk away. He couldn't be here at all.

"Did you get the kit open?" Karen asked Danny.

"Almost. Oh, my god. No. No. No, no. No, no, no I-I-I can't do this." Danny said to us and he had to. Especially if we weren't able to go down there ourselves.

"Danny, Danny, you can do this. We have one chance to bring you both up. In order to do that, you have to amputate Susannah's leg." Owen said to him. This wasn't going to end well.

"No. No. No. Oh, god." He said to himself and I was worried.

"Danny, you need to do this so we can get you both out." I said to him, it wasn't long before Owen got pulled away. Alex needed help. Who would've thought. Alex was being stubborn. I definitely didn't. But he came back after a while. A child had been found in the mess. Still alive thankfully. Trying to coach Danny through this was hard.

"Ok, now pour on that brown soap." Callie said to him and he did so. "And pick the thin silver knife. Good. Ok, now here's the thing. You're gonna have to really put your weight into it. Try for smooth cuts, ok?" She asked him and I was hoping he was strong to enough.

"Ok." He said to her.

"Now you see that area just below the tourniquet? That's where you're gonna cut. Do it horizontally, as deep as you can." Callie said to him and I saw him hesitate. She didn't need this.

"Are...are you...are you-are you sure she's not gonna feel anything?" He asked us and that was hard to say but she was unconscious.

"No, she's unconscious, Danny. She won't feel a thing." Callie reassured him and and now we prayed that she didn't wake up. Danny went to cut but stopped.

"I-I can't." He said to us. He had to.

"Danny...take that blue towel and cover her face. It'll help." Owen said to him and he did so.

"It might help you to not think that it's someone you love." I said to him, I know I'd have to do that if it was Jackson.

"Alright, now remember to put your weight behind it, ok? Here we go." Callie said to him. And she looked at us. "One...two...three..." she said giving him a count down.

"Oh, god! Oh, god! Oh, god!" He shouted and I hated listening to this. "Ah. I-I hit...I hit bone. I-I think I hit bone." He said to us and that was good news.

"That's good. That's good. Ok, put the scalpel down now. Now take the curved clamp and move it back and forth beneath the bone." Callie said trying to give him the next step.

"God. God, I-I can't. Uh, it-it won't..." Danny started before Callie cut him off.

"Put your hand in the incision to get more traction." Callie said to him and he was losing it. And now wasn't the time for that.

"I can't." He said to us, Owen and I both shared a look.

"Danny, I know this is hard, but we need to create more space so you'll be able to saw through her bone." Callie said to him. What made this worse was he was nearly done.

"No, I...No, I-I can't. Ok? This-this is my wife. All right? I-I'm gonna kill her. She's bleeding now." Danny said to us and I watched the screen.

"Danny, you're nearly there. Just a few steps left." I said to him. And he was.

"Danny, you can't stop." Callie said to him, we needed to get down there.

"No, you need to come down here. I can't do this. I won't do this." Danny shouted at us. He was giving us no choice.

"I'm going down." Owen said to us and I looked at him.

"I'm going with you." I said to him. There was no stopping me. I took my ear piece out.

"No. Dr. Hunt and Dr. Y/l/n, I can't let you go down there." Karen said to us, but she'd have to.

"He can't do it." Owen said to her.

"Yes, he can." Callie said to us. He couldn't.

"He can't. We are asking too much. We're going." Owen said to them and I was glad he was taking me as well.

"If it starts to go, I'm bringing you..." Karen started before Callie jumped in.

"I should go. I'm the orthopedic surgeon." Callie said to Owen as the two of were getting given the equipment.

"You have a wife and a child. We're going." Owen said as we started to walk away. I get that I had a child but my mum was around. Yes, I loved Jackson but he'd understand. As we were descending down not once did I ever think what I was doing was a mistake.

"Im...I'm sorry. Im sorry. Oh." Danny said to us and he didn't needed to be.

"Danny?" We both said to him.

"Im sorry." He said to us again.

"Danny, look at us. You're doing great. Now we need you...we need you to cover her head. Keep her head safe, all right?" Owen said to him and nodded at us.

"And Danny, you did good." I said to him as Owen and I got into position.

"All right., Owen said and we started to continue the procedure.

"I couldn't do it. I couldn't...I couldn't do it. I couldn't." Danny said to us but he did better than expected.

"Danny, you did it. You did the hard part. All right? Now, Danny...Danny, look away." Owen said to us and I looked at Danny.

"Trust me, it'll be for the best." I said to him and we cut through the bone. Until it was off.

"All right, she's free. Good job. We're gonna finish up, and we'll all be right behind you. All right?" Owen said to him and I nodded at Danny. We'd be up there soon. Danny kissed his wife's head and that was it.

"Take him up." Owen and I both said while looking at camera. We both then continued to pack it. Well this, I could write about. Once we got back to the hospital I didn't realize how dirty I was until I saw Jackson.

"Where have you been?" He asked me and I looked at him.

"I was in the sink hold doing a field amputation." I said to him and he crossed his arms at me.

"Sounds like you've had an experience. Just be glad you weren't here." He said to me and now I wasn't looking forward to anything. I then went to get cleaned up before going back to help Callie.

"What have you got?" Bailey asked us as she walked into the room.

"Uh, a field amputation with complicated extrications. Still assessing her multiple crush injuries, but I felt a broken pelvis for sure, and her BP's in the toilet." Callie said to her.

"Yeah, this is a Gunther." Bailey said and I was confused.

"Really?" Callie and I both asked her.

"Really." Bailey said to us. I wasn't expecting that.

"Think they can handle a Gunther?" Callie asked and I was still lost.

"Oh, I think they need a Gunther." Bailey said to us.

"Uh, what's a Gunther?" April asked us and I was glad I wasn't the only one that didn't understand. And I think I'd be finding out soon enough.
