Name: Crow/Blaze. Alright! The formatting you use to write the names is quite unique; I don't think I've seen it done like that! 

Name reasoning: Crow-her appearance | Blaze- her past personality, her right eye. Okay. 

Past names: Crow/Kit, Crow/Paw. Normal. 

Other names: Mountain, Black/Bird, Midnight/Raven (reasoning is in backstory) Wow, that's a lot of past names. 

Clan: MountainClan. MountainClan is mostly absent and prefers peace, avoiding major wars because of what happened. What happened was a prophecy, involving a cat from their clan, named Midnight/Raven. They live in a cave at the bottom of one of the tallest mountains. Their territory is large and involves a small part of two-leg territory. A waterfall is pouring out of the mountain next to them. Their hunting territory is spruce wood, but braver/bigger cats hunt near the mountains for bigger prey. That's cool! 

Past clan: none. Okay.

Gender: Female. Alright. 

Sexuality: Aromantic/Lesbian. Cool. 

Appearance: An extremely fluffy she-cat with bulky legs. She has curled ears and a white splotch on the left part of her face. She has a mutation called "muscular body that she inherited from her father. Her tail is long and puffed out. Her hearing is better than usual. Her left eye is a very pale blue that is almost blind. Her right eye is blazing amber. I am picturing a muscular cat right now, and it's so funny- I don't often see cats with mutations, and I've never seen an OC with a "Muscular Body"! And is her eye physically almost blind, or just looks blind? And what colour is she under the white spot?  

Personality: She used to be kind, carefree, peaceful and loving. But she is now cold and shut off. She puts barriers around herself and distances herself. She has extreme trust issues and if you ever show kindness towards her, she will get attached. She lost the ability to love, but she's recovering. She is overprotective and if you look past her cold looks, she's creative and funny. Balanced! I have an OC with a personality kind of like this; Hopepaw. 

Mate: None, but she has a crush named Bright/Blizzard. Bright/Blizzard—a pale cream Sokoke she-cat with hazel eyes and white eye markings, her white face fur being fluffier than normal. She has charcoal stripes on her face. Former member of AsterClan. She has a mutation called Seal Nose. I like her description! And the addition of quite a few of these characters having mutations is very unique. 

Kits: She's terrified of hurting them so she stays away from them so none. Fair enough.

Mother: Cinnamon/Apple—a cinnamon she-cat with small black speckles and a ginger mask. Her eyes are Dark brown and she has a mutation called "prehensile tail". Again, the mutation detail is really nice! But cats can't have brown eyes, let alone dark brown. Dark copper or hazel could be a better choice. 

Father: Coal/Web—a fluffy, black tom with a white brindle and blue eyes. He has a mutation called "muscular body". Okay, I see where Crowblaze got her mutation! 

Littermates:

Edge/Paw—a cinnamon tom with sharp, black stripes on his body. His eyes are dark brown and he has a mutation called "prehensile tail". Alright! Again, cats can't have dark brown eyes.

Thick/Paw—a fluffy dark-dusty tom with blue eyes and faded cinnamon legs. He has a strong, muscly tail that certainly hurts if you get slapped by it. Okay. 

Dusty/Paw—a dusty she-cat with brown eyes and a white left front leg. Her ears are slightly curved and she has excellent hearing. Cool!

Hard/Paw—a dark-dusty tom with harder paw pads and harder teeth. His eyes are grey and he has cinnamon stripes on his tail. Let me just say that Thick and Hard are two prefixes I don't think I've ever seen! They could be taken out of context, but that's alright, lol. 

Future cause of death: Suicide. Will stab herself in the throat with a sharp edge in the mountains. Brutal. I assume the reason is in the backstory, but an explanation here would also be nice.

Rank at death: Senior warrior. Okay.

Past ranks: Kit, Apprentice, young warrior, warrior, senior warrior. Cool, a pretty fulfilling/long life. 

Good traits: Creative, intelligent, tactical, funny, humble, secretly caring, logical, observant, secretly caring But only if you get through the barrier. Alright. 

Bad traits: Cold, critical, distanced, merciless, too loyal, bad at understanding emotions, sometimes evil thoughts, obsessive over loved ones, possessive, jealous, not resistant to pressure, not responsible, quiet to the point she won't speak for whole days. Maybe a few more good traits to balance with the bad.

Okay! Pretty balanced! 

Likes: The stars, quiet places, the moon, darkness, birds singing, the sound of water, calm places, Bright/Blizzard. Fair enough!

Dislikes: Sweet things, mean cats, loud places/noises, war, someone hurting her loved ones, disrespectful cats, fear, insecurities. Again, fair enough.

Scars/Injuries: A deep and ugly-looking scar stretching from her left eye to her back legs. Wow, that's pretty extreme and big. How did that happen?!

Backstory: She was born into a normal family, to Cinnamon/Apple and Coal/Web. She was happy with her siblings and contributed to her clan as much as she could. She slowly developed a crush on a fellow apprentice named Bright/Paw, that came from AsterClan, a neighbouring clan. A prophecy came to the medicine cat that involved Bright/Paw, and probably her. Cross Clan relationships and having a prophecy involving the Main Character is a bit generic and overused, but if they are written and explained well, they can still definitely work. And why would a prophecy involve two apprentices from opposite Clans? What was the prophecy, and how big and important was it? 

 However, her life was torn apart when her ambition grew and she couldn't stand being a background character. She leaves home, killing her leader, Dawn/Star to the point he was left with two lives. ...That escalated VERY QUICKLY. Why did she kill Dawnstar? What beef did she have with the leader? And how old was she when she did this? An apprentice or young warrior would have an extremely tough time draining multiple lives from a seasoned, and trained leader. And why did she feel like a background character? Surely she could have done something to make herself feel important. 

After taking a break from her travels and resting under the moon, she encounters her clan founder, named Mountain. Mountain informs her that she is part of a generation of reincarnations. Mountain's reincarnation. All the cats that looked like her, which were Black/Bird and Midnight/Raven, were reincarnations of Mountain, and so was Crow/Blaze. Mountain was afraid of death so she used reincarnation to escape. Bright/Paw, who was renamed Bright/Blizzard, was Aster's reincarnation, Aster being AsterClan's founder. Aster and Mountain were forbidden mates, and Mountain ended up a villain while Aster ended up a hero. Being a reincarnate is a cool concept! How old are Mountain and Aster at this point? Are they ghosts, or still living? 

Startled by this, she goes back home to tell Bright/Blizzard about this. She reaches home, but she's forced to fight her mentor, the deputy, named Dusk/Song. She wins but doesn't kill her ex-mentor. She manages to warn Bright/Blizzard and they run away to warn the leader of AsterClan too. Crow/Blaze's impulses win and she declares war over MountainClan. Wait, why? Why did she have to fight Dusksong, and why did she declare war on her home Clan? What the heck did they ever do to warrant that sort of aggression? How long was this war? 

She barely wins the war with them, but only because Bright/Blizzard joined her and her family refused to fight her. She fights AsterClan, and as she is about to lose, Bright/Blizzard drags her out and heals her. Bright/Blizzard manages to convince Crow/Blaze to turn over to the good side. Crow/Blaze then lives the rest of her life in guilt. (This part will not be included in the book so it's safe to read) When she becomes a senior warrior, she can't deal with the guilt and ki//s herself by finding a sharp edge in a cliff and stabbing her throat with it, letting a new generation of reincarnations begin.  Okay... How did she win the war? She's one cat, two counting Brightblizzard. How big was the Clan? Did she stay in the Clan, or keep running? Surely her Clan wouldn't trust her anymore, and wouldn't let her in. And after so long, I would think the guilt would have faded enough that she wouldn't feel the need to... Take herself out. 

Overall, Crowblaze is a cool OC! She's got quite a few flaws in her backstory, but with those sorted out, she'd be pretty perfect! She gets a 7/10 for now.
