Hazel was taken by M.E.C.H, when Optimus and Hazel was doing a patrol, but M.E.C.H men ambushed them, Hazel never knew about M.E.C.H plans, and when they started to experiment on her after Hazel was kidnapped, because of her being part human and robot.

Hazel almost killed them, because she was scared, they continued to experiment on her for months, until she escaped.
