So this is pretty simple! For a form, just comment on the idea you want:

Mention location: (say place where mention will be)
[If a book idea] Would you like me to make you a cover?: Yes/No
Estimated release of book/character: __ weeks

I hope that's easy enough! As for organization, each chapter will contain the title of the book along with a letter or two in parenthesis before and after it. Here is the key:

Before title

O: Open

P: With corresponding plot [settings only]

C: Closed

After title

W: Warriors

D: Wings of Fire

O: Original world

WO: Warriors/Wings of Fire but can be tweaked to become an original
