I can deny your request.
You have to fill out all the required details of the form.
Do not fill out a form if it's closed, else I might miss it when I reopen because I will not look at forms while I'm not taking requests.

Really all this is is that I ask that you credit me in the description of your book. That's all, I don't need you to follow or vote or anything.

And don't feel bad asking me to change something or even make you a different cover! Really I don't mind all.

One thing though, please, if I make you a cover use it at least two weeks and please don't ask anyone else to make one at the same time for you. It's rude to both of us- covers take time. Yes I enjoy doing it but that doesn't make it easy. To see hard work just tossed to the side isn't cool.
