IMPORTANT A/N: Hey everyone! Before you read this chapter, there is something I want to bring to everyone's attention! Dr. Smiley will have a very different appearance in this chapter, he won't be described as everyone knows him. But why?
Because that's not Dr. Smiley.
I want to bring this to the attention of as many people as possible. And I wish to thank Jordan Persegati for bringing this to everyone's attention first.
BUT THE FANART YOU SEE OF DR. SMILEY IS STOLEN. THE ORIGINAL DESIGN IS ACTUALLY AN OC THAT BELONGS TO Ko-yamii ! If you want more info, I recommend you watch either her video on Youtube, or Jordan's, because they both cover the issue.
However here is a summary; the creator of Dr. Smiley never came up with a design for him and wanted to find one, so they came across Ko-yamii's OC and decided that was good enough and stole it, uploaded it without crediting and claiming it to be Dr. Smiley. Everyone believed them.
But now, hopefully, no one will. Because Art Theft is a crime and I will not stand for it. So please everyone, spread the news, and lets all try to come up with our own designs for Dr. Smiley! I'll do my best to make up my own in this story as well! And don't worry, he's still the same character!

You were sick. It was something that had been haunting you for your entire life. Or, most of your life. As long as you could remember anyway. Having sick kidneys and continuously needing treatment can really blur the lines of time when you're continuously going to the hospital and back home, every month, week, day.
Today you were on your way to the hospital once again, for a reoccurring check-up. Your kidney disease had gotten worse, and you were feeling it slowly taking over your body. You were weak, unable to do usual things most humans did on a daily basis. In need of machines just to help you cope, live. It was stressful, it was draining, but you were working on fixing the problem, there was hope. You were next on the waitlist for kidney donations.
The only downside was... You might never get them. People weren't known for donating their organs, it was something hardly anyone even thought about. And it was a bummer, but there was nothing you could do. All you could do was wait and hope, attend your appointments, rest, try not to push yourself.
It didn't help that today, your parents were both at work, your older sister was away for the next few days. So your grandmother had to drop you off at the hospital. And better yet, she had an appointment later she couldn't miss either, so you had to take public transport home.
It wasn't too bad, as long as no one bumped you or knocked you over, or you didn't get sick or dizzy, or you needed the bathroom, or you got hungry or thirsty. Aside from that, you should be fine. No biggie.
In fact, you had just finished your appointment, and were currently waiting at the bus stop for your bus. Glancing at your phone, you saw it was due to arrive in five minutes. Letting out a sigh, you pocketed the devise and leant on the pole. Five minutes was a long time to wait, hopefully the bus isn't late either.
A hot-cold feeling suddenly took over your body and the world started to spin. You let out a groan, sitting back on the bench and holding your head. Great. Hopefully the bus doesn't just keep driving, hopefully the bus knows this is a hospital and people might not be able to hail for it to stop, hopefully you weren't going to be left at the hospital for another 30 minutes.
You could hear the sound of a vehicle driving up, and you raised your gaze to see the bus speeding towards the stop. You barely managed to raise your hand, seeing the bus indicate and slow to a stop. The door opened and the driver turned to look out at you,
"You alright luv?" He asked, flicking his cap out of his eyes.
"Need... To get... To (insert home here)..." You mumbled, and the driver got out and walked over to you.
"Yup, lets get you onto the bus then." He said and gently took you by the arm, "Can you stand?"
"Yeah..." You mumbled, the hot-cold flush disappearing slowly as you got back to your feet. Finally able to focus, you smiled at the driver, "Thank you for stopping... And helping me."
"Lots of people at this stop need help. I wouldn't be doing my job if I just drove by." The driver chuckled, before helping you into the bus. Once you had paid, you sat down near the front and leant onto the cool window. The bus took off, and you hardly noticed how rough the drive was.
Almost instantly, you felt eyes on you. Someone was staring, possibly even the whole bus was staring. But that was something you hadn't done when you got on, you had no clue how full the bus really was. Whoever was staring though, they were staring hard, their eyes were burning into your shoulder, and you felt the urge to turn around.
And your eyes connected with that of a piercing green. Large green eyes that bore into your soul, looking slightly crazed and bloodshot. The guy was wearing a mask, it looked like it had a strange red blotchy pattern on it, funnily enough looking like blood. He had long, snow white hair that fell in front of his face, making him look sick, as it made his pale skin look... Well, he looked like a ghost.
Aside from that, he wore a long white jacket over a buttoned-up shirt, a red tie hanging from his neck, along with black slacks that covered his legs and dress shoes to match. Was this guy a doctor? You'd never seen him before, and wondered if he worked at the hospital, or another one. He tilted his head at you, before standing and making his way over.
You blinked, before a pang of anxiety went through your chest and you turned your attention to the front of the bus. Oh God was he making his way over to you? No no, surely he was just getting off the bus right? That's why he's walking up to your seat right now. Oh God he's stopped next to your seat, his hands are in his pockets, and he's facing you. What do you do?
"May I sit here?" He asked, his voice low and smooth. You glanced up at him slowly, feeling your body shaking. This dude was creeping you out but, he closed his eyes as if he was smiling, "I mean no harm, I want to help you."
You nodded slowly, before shifting over and giving him the space to sit. Once he was next to you, he let out a sigh, staring at the bus driver, "From the dark circles under your eyes, and the sickly colour of your skin, I can only imagine you are extremely sick, possibly a disease in need of a new organ?"
What? How was he able to guess that? You glanced over him again, he did look like a doctor...
"Kidney disease..." You huffed, "I'm next on the waiting list actually."
"Well that's good news." The doctor nodded at you, his eyes were still burning your soul, his emerald green...
Shaking your head, you faced the front again, "Yep... Just waiting now."
"That's how life works I suppose." The doctor said, also facing the front, "Waiting... Always waiting... For a phone call, a special day, death... Life is such a patient thing, isn't it?"
"Y-Yeah..." You gripped onto the pole in front of you, this guy was making you extremely uncomfortable.
The bus driver could tell as well. He glanced up and pulled the bus too a stop,
"Hey, would you mind leaving the girl alone mate? She looks drained enough as it is."
"... Right... My apologies." The doctor nodded, before standing and walking back to his seat. However, for the rest of the ride, he never took his gaze off you.
Finally, the bus reached your bus stop, and you jumped off, glad to be out of the view of that stranger. He gave you the creeps, and you were thankful he didn't know where you lived. You headed home, where you rested for the rest of the day.
The next day, your parents had both left for work again, leaving you home alone. They had made sure you had everything you needed, they always did, they worried about you. So you got used to looking after yourself while your parents worked to try and pay off the cost of your bills. You were thankful they were so caring, and thankfully you understood why they had to work so much. You just wished they didn't have to work to the death just to pay for your health.
But don't worry, you smiled to yourself, thinking about how it will soon be all over. You'll have a new kidney, and you won't have to worry about any other costs after that.
There was a knock at the door, and you sat up from the couch. Someone was here? Why would anyone be here? Unless one of your parents ordered something and forgot to tell you, there was no reason for people to knock on your door. You frowned to yourself, before making your way to the large wooden door.
The figure on the other end made you jump. It was the creepy dude from the bus yesterday. He stood tall, almost reaching the top of the doorframe, and he leant over you as you opened the door, the mask still on his face.
"Hello (Y/N)." He greeted, eyes unblinking, and you stepped back, "Sorry for the rude interruption, I just couldn't stop thinking about your condition, and I felt I should help."
"H-How..."
"Your records at the hospital." He said, "I only wish to help, I can do it all for free, and you'll be free of this horrible disease."
Okay, this was creepy as fuck. You were immediately uncomfortable. What do you even say? He only stared at you, waiting for a response. You couldn't even close the door on him, he was leaning inside to stop you, it was obvious he had already made the decision for you.
Letting out a sigh, you lowered your head, "I can't say no, can I?"
"Well of course you can." The guy nodded, "But I do insist you take up my offer, it really is life changing."
"U-Um.." You rubbed your arm, knowing you should say no but... "If it helps my parents then... Alright."
The thought of lightening their pay load made you hopeful, maybe one day they'll be able to spend time with you as well, once everything else has been paid off. A small smile appeared on your face and you nodded,
"Yeah, I'll do it then."
"Excellent." The doctor's eyes squinted, as if he was smiling, "Now, let's head to my apartment, all my tools are there."
And that should have probably been the seventh red flag.
But you followed him anyway, eventually ending up at his apartment, which was located in a rundown location. Any other building in the area would have been abandoned ages ago, it was a ghost suburb. He lead you to the front door, opening it and stepping aside to let you in first. You glanced at him, before slowly making your way into his home, and what you saw would scar you for the rest of your life.
His living room was dirty, his couch stained with blood, jars full of murky water and... Things... Lined the walls, dirty clothes were heaped in the corner of the room, next to a small box-like television. Just glancing at the different sorts of garments, you knew they weren't his. They were also bloody.
"Wh-What exactly do you do...?" You asked, turning to see him close and lock the door.
"I help people, I cure them." He said, looking around the room, "I take specimens from them for study."
"R-Right... I uh... I think I might have changed my mind..." You said, stepping away from him, trying to find a way out.
"That's to bad..." The doctor said, his eyes wide and staring at you. His entire demeanour changed, and he seemed almost animalistic, "Why must you make this harder for us both?"
You frowned, before taking off in the opposite of the direction of the front door. Without thinking you ended up in the kitchen, even more jars of organs and 'specimens', the windows were barred with wooden planks and tape and you growled to yourself. Crap, time to try another room. But the only way out was through the living room. And the doctor was standing there, a needle full of a strange liquid in hand.
"Come on now, I'm going to help you, I'm going to cure you."
"Cure me from what exactly?"
"From life." He said, his head tilting to the side, "Everyone is always trying to run from it, but there is no point. They value life, so I wish to make them value death as well. It's inevitable either way. Why can't you humans just value death?"
"Us humans..? What are you saying?" You asked, completely bewildered, and he was suddenly standing in front of you. His hand gripped your arm, making you flinch and try to fight back, but it was no use, his hand was most likely bruising your skin the more you fought.
"Just stay still..." He said, leaning down with the needle.
"N-No!" You growled and swung a clawed hand up at his face, pulling his mask off. It dropped to the floor, and you were left staring into the wide, sharped tooth grin of a monster. He chuckled, before stabbing the needle into your arm,
"Let's try this again shall we...?" He asked, licking his lips, and as much as you struggled, you couldn't fight the darkness that pulled at your vision, "Doctor Smiley is back."
AHHHH I'M SORRY THIS SUCK, I WROTE THIS LITERALLY TONIGHT- I'VE BEEN SO BUSY WITH UNI THAT I HADN'T WRITTEN THIS YET AND I NEEDED TO SO HERE IT IS.
I STILL HOPE IT'S SATISFACTORY AT LEAST
