Here is the form for everything. And some rules I need you guys to follow.

Rules:
1. NO anime or Supernatural requests (I'm not so good at making those)
2. NOTHING with falling in love with my kidnapper or anything like that.
3.You MUST give me credit in the description of your story or first part of your story.
4. DO NOT rush me, I will give deliver your cover when I have the time. But it won't take that long, so don't worry. (usually 2-3 days, it depends)
5. I get to deny your cover if your too vague and don't fill out the whole form. (most likely won't happen) PLEASE be as descriptive as possible. It makes it easier for me to make the cover that you'll like. And to make my life easier, just request in ONE comment and not all separated.
