If you want to apply for me to make you a cover, here's the application form! To make sure this cover includes everything you want, please full out the form below to the best of your ability. (It's super easy, don't worry:)

TERMS OF USE AND APPLICATION (RULES)

- You must credit me in the description of your story if you use the cover.
- I am only making covers for Wattpad books!
- If the story is cross-published on a different site and you would like to also use it elsewhere, that is fine (You will still need to credit me as '@stormysky- on Wattpad'). 

I will not accept applications for works depicting:
- NSFW
- shipping of real people (actors, streamers, Youtubers, musical artists, etc.)
- any form of abuse, r*pe or s*xual harassment
- the glorification of suicide or self-h*rm

These are my boundaries, and I will reject applications that go against my boundaries; if your work includes one of these, I advise you to find a different designer. This list may update in the future.

3. Link to book if it's published. if it's not published, please give a summary of the story. you can also pm the summary if you want to keep it private.

4. Name(s) you would like on the cover (like, who is it by? Most of the time it's your username or pen name)

5. Any specific icons/photos you want in it? If you have certain ones in mind, please link the image(s) here.

6. Subtitles? (Short blurbs of text on the cover)

7. What's the mood of the story? Genre?

8. Anything else you know your cover MUST have? (for example, "calligraphy title font" or "yin and yang")
