An actual wormhole would be analogous to this, but with the spatial dimensions raised by one. For example, instead of circular holes on a 2D plane, the entry and exit points could be visualized as spherical holes in 3D space leading into a four-dimensional "tube" similar to a sprinter.

Another way to imagine wormholes is to take a sheet of paper and draw two somewhat distant points on one side of the paper. The sheet of paper represents a plane in the spacetime continuum, and the two points represent the distance we want to travel, a wormhole could connect these two points by folding that paper (⁠the plane) so the points are touching. In this way, it would be much easier to traverse the distance since the two points are now touching. So if I am here in this Null chamber that is billions of years far from Earth, Manhattan, New York, the wormhole bends that distance and turns it into mere meters, wormholes can bend space. Truth is, yet theoretical, some scientists believe that even time travel is possible with the use of wormholes, future time travel of course. Past time travel would be theoretically possible but it will have no impact on the present because of the Grandfather Paradox. The grandfather Paradox is a paradox that states that if a guy miraculously can travel back in time and decides to kill his grandfather before him being born, then the death of his grandfather would cause him to never exist, which will lead to him never being able to go back in time and kill his grandfather, get it?
